Job Description:

Summary: This position works across all 3 of our Assertive Community Treatment Teams. They will assist clients in developing amicable relationship within the community. This position will provide support services to clients transitioning to the community.

Essential Duties and Responsibilities:


  • Collect pertinent data needed for utilization review

  • Provide feedback on treatment plans

  • Formulate appropriate post-discharge placement plans that include services for client and family

  • Complete discharge summary and other paperwork

  • Develop appropriate discharge plans collaborating with referral sources

  • Provide transportation for clients to access community resources

  • Emergency services duty on rotation that may include commitment procedures, after hour assessments, crisis planning, and hospital diversion

  • Other duties as assigned.

Required Qualifications

Education, Licensure, & Experience


  • Bachelor’s Degree Required. Minimum of 1 year of experience working with individuals with severe and persistent mental illness.

  • High School Diploma with a fully licensed CADC. Minimum of 1 year of experience working with individuals with severe and persistent mental illness.

  • QP status according to 10A NCAC 27G.0104 or Certified Peer Support Specialist preferred

  • Experience with vocational / housing support preferred.

Physical Demands

While performing the duties of this job, the employee is frequently required to talk or hear. The employee is regularly required to stand, walk, climb stairs, use hands to finger, handle, or feel, and reach with hands and arms. The employee is occasionally required to sit, stoop, twist, kneel, or crouch. The employee must occasionally lift up and/ or move up to 25 pounds. The employee must seldom carry up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

Work Environment

The noise level in the work environment is usually moderately quiet & indoors. This position will require local travel. This position will be scheduled based on operational need & will be required to provide own transportation. Incumbent may be exposed to virus, disease, and/or infection from clients in the work environment. Incumbent may be exposed to traumatic situations (i.e. psychiatric).
