Evaluates patients, plans and implements an effective Physical Therapy program based upon physical assessment, clinical judgement, and psychological sensitivity for inpatients and outpatients with diverse diagnoses; provides patient education, consultation, and discharge planning; supervises and delegates patient care to Physical Therapist Assistants, and Rehab Technicians.

PATIENT ASSESSMENT/TREATMENT: Performs patient evaluations and treatments in alignment with the Physical Therapy Practice act and departmental policies and procedures to assess patients functional deficits and identify recommendations for intervention utilizing basic skills with occasional supervision throughout the entire span of life with minimal supervision. A. Interprets physician referrals to identify rehab indications, contraindications, and precautions for all diagnoses throughout the entire span of life with minimal supervision. B. Conducts and interprets patient evaluations to determine functional deficits utilizing appropriate basic evaluation skills. C. Identifies patient and therapist goals for maximal function and quality outcomes that are objective, measurable, and time specific. D. Develops, implements, and progresses a treatment plan choosing appropriate techniques that incorporates patient and therapists goals to improve function and provides age specific treatment. E. Documents evaluation and treatment findings to meet legal and reimbursement requirements and completes necessary paperwork for communication in compliance with department policies and procedures.


COMMUNICATION: Exhibits effective patient management skills to enhance comprehensive patient care and continuity. A. Administers physical therapy programs by considering patient safety, functional outcomes, and patient's physical, mental, psychosocial, cultural, and financial needs. B. Provides comprehensive information to all personnel involved to assist in continuity of care. C. Demonstrates ability to assign involved to assist in continuity of care. C. Demonstrates ability to assign priority to all or part of a patient's treatment without compromising quality of treatment. D. Ensures timely and appropriate discharge of patients by participating and planning for patient's discharge. E. Provides continuity of care by instructing patients and/or family, and community in exercise programs, home programs, body mechanics, etc. and adapts education material to audience's mental and physical capacity F. Promotes teamwork by actively participating in team activities, and fostering effective working relationships with coworkers and supervisors. G. Promotes effective working relationships with coworkers and supervisors by communicating effectively, resolving conflicts, and offering assistance. H. Demonstrates commitment to self managed behavior by accepting new challenges and initiates opportunities for growth.


DOCUMENTATION: Performs required documentation for evaluation and treatment findings and communicates this in a competent, complete, accurate, clear, concise and timely manner to meet patient and/or family needs, legal and regulatory requirements. Completes charges and billing documentation following managed care and insurance guidelines to ensure maximum reimbursement. Maintains confidentiality of patient, employee and department information to ensure patient rights. SERVICE EXCELLENCE: Assures customer satisfaction by exhibiting service excellence skills. A. Exhibits customer service by responding to patient or family inquiries, complaints; greeting patients warmly, politely, and fostering a positive attitude; displaying care, concern, cooperation and respect for patients, families, and coworkers. B. Uses proper telephone etiquette by answering phone calls identifying self and department and offering assistance. C. Ensures patient's rights by maintaining confidentiality and privacy during evaluations and treatments.
