RN - The Move Forward PIPP Leader




OBJECTIVE: The MOVE FORWARD PIPP Leader is a newly created position. The PIPP Leader will utilize clinical assessment skills, competencies, best practices, and emerging technology to formulate interventions and strategies to support resident mind health and mobility. This position will also provide staff training for Implicit Bias and Resiliency.



Site: This PIPP Leader will be working at Anoka Rehab and Living Center, 40 hours a week!



ESSENTIAL FUNCTIONS:



The responsibilities include the following:


  • Utilize clinical assessment and critical thinking skills for MOVE FORWARD Program tools.

  • Provide program education to staff, resident and family members.

  • Lead the MOVE FORWARD Teams and provide direction, support and resources as required.

  • Conduct facility resident and staff Resiliency Groups.

  • Conduct and coordinate the Staff Bias Awareness Tool and assess and analyze findings with the MOVE FORWARD PIPP team and implement appropriate interventions with leadership.

  • Interact with multiple stakeholders to deploy program goals and assess, align, and implement interventions.

  • Interpret and analyze facility QIs and QOL measures and assess and develop interventions for on-going improvement and sustainment.

  • Partner with the Empira Liaison and SMEs to successfully deploy program in the facility.

  • Develop and nurture a trusting relationship with all stakeholders.

  • Report to the multiple stakeholders on program goals and outcomes including; but not limited to: QAPI, IDT, Medical Provider Group, Resident and Family Council.

  • Effectively use program tools including assessments, audits, educational resources and technology.

  • Implement Somatix monitoring program and utilize assessment findings to improve resident care and quality of life.

  • Ability to work flexible hours to accommodate all stakeholders.

  • Ability to travel 10% of the time for program needs.

  • Attend required meetings and trainings.

  • Other duties as assigned.

  • Is aware of safety issues and strives to accomplish all responsibilities in a safe manner.

  • Accepts and follows through with all policies and procedures.

  • Identifies and corrects hazards and unsafe acts and reports all quality issues.

  • Is aware of and adheres to all HIPAA regulations.

  • Adheres to organization's policies on infection control, safety, and resident rights.

  • Completes all Relias and other training required.

  • Identifies compliance concerns, reports to supervisor or the Compliance Officer/Compliance hotline.

  • Participates in facility QAPI, Quality Assurance Performance Improvement program and activities.




Additional:



  • Eligible to work at a site that receives federal funding.

  • Possess skills in problem definition/resolution.

  • Has regular and reliable attendance.

  • Participates in required in-service training sessions.

  • Participates in facility committees as directed.

  • Performs other tasks as directed.

QUALIFICATIONS:


  • Registered Nurse with active Minnesota Licensure.

  • Proven leadership skills to lead and work with staff, residents and families.

  • Proven adult learning and teaching skills.

  • Proficient computer skills with Microsoft Office applications and other required software programs.

  • Must be able to function independently and demonstrate flexibility.

  • Excellent interpersonal, communication, and organizational skills both written and verbal.

  • Ability to manage multiple tasks/projects and deadlines simultaneously.

  • Ability to maintain confidential information.

  • Understand, interpret, and utilize data to improve resident care.

  • Competency utilizing quality improvement tools and software.

  • Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
