Central Ohio Primary Care is seeking a full time Operational Support Coordinator within our Hospitalists group. This position is directly involved in a collaborative process with the Inpatient Support Center (ISC) that primarily supports the Transition of Care Nurse program (TCN). The Operational Support Coordinator communicates and completes tasks that will enable the Transition of Care Nurses (TCN) to spend more time with patients, allow for an expedited workflow and support communication with primary care team. This role is office-based and requires working primarily within multiple electronic medical records (EMR). The schedule is 4a-12p Monday- Friday and is located at our Dublin Methodist Hospital office.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
• Track all hospitalized COPC patients. Communicate with Primary Care Physicians and Care Coordinators as needed.
• Notify Primary Care Physician of all hospitalized patients through health record.
• Assist in daily TCN list-making using multiple hospital medical records.
• Foster a climate allowing for direct communication between the Care Coordinator, patient, and appropriate care providers, to optimize outcomes.
• Communicate often with outpatient Physicians and Care Coordinators utilizing various communication modalities.
• Facilitate provider contact and care team appointments as needed to coordinate patient’s care needs across the continuum of care.
• Assist with FMLA paperwork and prior authorizations.
• Other duties as assigned.

A. Education, Licensures & Certifications
Required: high school diploma
Required: 5+ years’ experience working within electronic medical records
Required: 5+ years’ experience data entry
B. Knowledge, Skills & Abilities
• Knowledge of outpatient practices, the hospital setting and the post-acute space
• Strong knowledge of Electronic Health Systems
• Ability to multitask
• Ability to adapt to a changing work environment
• Ability to work independently with little supervision
• Ability to develop and maintain spreadsheets in Excel
• Strong analytical, organizational and time management skills
• Heightened attention to detail
• Excellent written and verbal communication skills, especially the ability to communicate effectively in stressful situation
• Self-disciplined, energetic, passionate, and innovative
• Confidence to communicate and outreach to other community health care organizations and personnel
