Job Summary

Point32Health is currently seeking highly motivated individuals to join our organization as a Medical Social Worker. This role is responsible for the provision of clinical and concrete services and provides consultative care management and supportive services to members with medically and behavioral health related psychosocial needs. The Medical Social Worker collaborates with NCM, providers, and external agency staff, to coordinate services. This position also requires providing clinical expertise to Care and Disease Management staff in the establishment of care plans and in the development and administration of Care Management programs. The role is primarily remote, with occasional on-site meetings in our Canton, MA office, as well as occasional in-home visits to high risk members.

Job Description

• Provides medical social work services for members telephonically, acting as a liaison between the care manager, other treatment team members and community agencies and providers. This includes formulation of an assessment of social and psychosocial needs, providing social support around coping, address mandated reporting concerns, development and implementation of treatment plans, referral to programs and agencies and monitoring the success of the care plan.


• Develops and maintains thorough working knowledge of public and private community-based agencies and services. This may include developing and maintaining collaborative relationships with agency staff to facilitate member access and coordinate services.


• Collaborates with Care Management staff to identify and provide ongoing educational information concerning medical social work services and community resources and programs.


• Acts as a resource, provides clinical training, and uses multiple educational approaches to assist care managers with management of members experiencing psychosocial stresses and barriers to care.

Requirements

• Master of Social Work, LICSW licensed required (in the state of MA preferred)


• Minimum of 5 years' experience in human service/community sector in the provision of social work services, with a minimum of 2 years' experience in medical setting or in a multidisciplinary setting focused on care of chronically ill/disabled population


• Minimum of 1 year experience working with Behavioral Health related programs


• Excellent interpersonal skills, oral and written communication skills and proficiency with Power Point, Email, Word, Excel and Internet


• Well-developed assessment/problem solving skills
