Location and hours: We are hiring at our Downtown Duluth clinic location. Flexibility to work remotely and in-person with clients, based on client & team needs between the hours of 7AM-6PM, Monday-Friday.

Employment Type: Full-Time or Part-Time.

This position is a permanent hourly role and not dependent on grant funding.

Program Description: Behavioral Health Home (BHH) is a monthly service that provides case management for individuals with a mental health diagnosis & MA or MA/PMAP, in the community. It's a lower level of care and considered a step-down from ARMHS, Targeted Case Management, or Assertive Community Treatment. BHH is a team-based model that includes a case manager, nurse & either a peer support specialist or community health worker.

The CHW, Nurse & Case Manager will work with clients on goals related to:

Connection to preventative care
Connection to mental health services
Connection to community programs such as housing, food shelves, employment, etc.
Transitions in care

Nystrom & Associates is seeking a Community Health Worker to join our Behavioral Health Case Management Program.

In this role, you'll have the opportunity to:

Provide culturally responsive, holistic care in the community.
Work closely on a team with nurses and case managers.
Interact with clients on the phone or in-person monthly.
Provide clients with health promotion services by assisting them with skills related to overall health and wellness.
