We are recruiting for a skilled clinician with comprehensive acute care and homecare experience. The RN Team Leader will work (24 hrs. per week; 4 hour afternoons from 12p-4p Mon, Tues. Thurs. Fri. and 8 hrs. Wednesdays. Ability to provide triage support through assessment and symptom management including supporting field clinical staff, review of physician orders assisting them in care coordination. Excellent communication, presentation and teaching skills required. The Team Leader will assist Clinical Homecare Managers in developing, maintaining and monitoring all standards and outcomes for the Homecare Division. Acts as a liaison for patients between homecare and hospice divisions. Reviews clinical record documentation to evaluate compliance with standards, policies and procedures in collaboration with the Homecare Managers and Clinical Support Specialists. We utilize Netsmart for our EMR. Team Leader will share in agency recognized holiday rotation working one summer and one winter holiday with occasional weekends. Holidays pay time and half and weekends pay $4.00 an hour differential.
Qualifications:
Minimum of Associate’s degree in Nursing
Current professional license with New Hampshire Board of Nursing
Three (3) to five (5) years of clinical experience
CPR Certification
Medicare (MCA) and state regulations proficiency
Prospective Payment System (PPS) interventions in relation to patient outcomes proficiency
Preferred
Bachelor’s degree in nursing (BSN)
Extensive homecare experience
Extensive triage and scheduling experience
