Responsibilities

The Program Monitor is responsible for providing support services to individuals residing in a nine-bed community home for youth aged 18 to 22 who exhibit challenging behaviors due to trauma-based diagnoses, sexually abusive behavior, cognitive disabilities, and/or intellectual challenges.

Program Monitor Duties/Responsibilities:


  • Implement Activities of Daily Living, Individual Service Plan Objectives, and Behavior Plans.

  • Provide job coaching, career development, and prevocational support services; including teaching and training, specific to each individual’s needs.

  • Promote a high quality of living in all areas including medication administration, meal preparation, housekeeping, personal hygiene, and leisure/recreation.

  • Provide transportation of individuals to scheduled appointments or social activities.

  • Provide off-site supervision, as necessary, when individuals are engaged in volunteer or employment opportunities.

  • Record progress notes, behavioral data and log notes for each individual residing in the program.

  • Ensure the safety of the individuals and general public.

  • Model appropriate behavior for individuals residing in the program.

  • Engage in the upkeep of the residence.

  • Attend all required training designated by the Program Director, Assistant Director, and/or CRJ.




Qualifications


  • Willingness to work flexible hours, good communicator and able to organize and be proactive, maintain order and follow through with required responsibilities while maintaining client confidentiality and respect for clients and co-workers at all times

  • Good interpersonal skills with effective time management skills

  • Valid Massachusetts Drivers License

Program Monitor Qualifications Required:


  • Minimum of High School diploma or GED.

  • Prefer experience in residential settings.

  • Prefer experience with system-involved youth.
