Job Description:

In keeping with the Mission/Vision/Values of Centerstone, a CCBHC Counselor is responsible for conducting screenings/assessments; facilitate evidence-based interventions; provide consumer education; communicate/collaborate with community providers and family members; and assist consumers/families in navigating healthcare system. The CCBHC Counselor may also be responsible for assisting with linking clients to appropriate services, advocating on their behalf, providing support and monitoring effectiveness of services as needed. The CCBHC Counselor is required to use independent discretion and judgment when providing services to clients. As part of Centerstone's team-oriented philosophy, the CCBHC Counselor is expected to actively participate in a variety of cross-functional teams and performance improvement initiatives.
