Job Summary

Works within the vision, mission and philosophy of the agency. Exhibit behaviors consistent with the Service Principles. Demonstrates and develops supportive/coaching relationship with patients and their caregivers to help them navigate the system to support their well-being. This includes providing assistance (e.g., attending appointments, providing community linkages) and supporting education related to physical health, behavioral health, and social determinants of health. Supports the whole person care principles.

Essential Duties

1. Works actively with the program’s other team members to develop strategies for outreach to consumers who tend to isolate themselves.

2. In coordination with a multidisciplinary team, assist consumers in selecting services and/or programs that will be useful in achieving their recovery and/or treatment goals.

3. Demonstrates a commitment to consumer choice and empowerment.

4. Demonstrates an ability to develop respectful interpersonal relationships with consumers while maintaining appropriate boundaries.

5. Assists and supports consumers with identifying, linking to and appropriately utilizing healthcare resources including the member’s primary care physician as well as specialty care providers, dental care and vision care, if applicable.

6. Assists and supports members with establishing benefits such as Supplemental Security Income (SSI), Medi-Cal and other benefits for which they are eligible.

7. Assists and supports members with identifying their housing needs and/or resources in the community to meet these needs.

8. Assists and supports members with linking to and participating in substance abuse services both on-site, if available, as well as in the community.

9. Assists and supports members with linking to and utilizing education and employment services.

10. Assists consumers in learning social skills and independent living skills that facilitate community. Integration including budgeting, transportation and grooming and hygiene.

11. Supports consumer’s integration into the community through social and recreational activities.

12. Develop and facilitate peer support and self-help groups as appropriate to recovery/treatment goals.

13. Helps consumers and family members identify, understand and combat stigma and discrimination associated with mental illness and develop strategies to reduce self-stigma.

14. Increases awareness and provide linkage to self-help programs and consumer advocacy organizations that promote recovery.

15. Supports the vocational choices consumers make and assist them in overcoming barriers to employment.

16. Encourages and supports members’ skills and talents.

17. Maintains billable services consistent with program needs and funding source expectations, through documentation and other required paperwork.

18. Participates in off-site presentations introducing the program to community partners and potential new consumers.

19. Attends and participates in staff meetings on a regular basis to provide input towards program development and staff training.

20. Attends and participates in scheduled supervision to provide or receive input towards program development and staff training.

21. Exhibits a non-judgmental approach to effective listening maintaining, respectful eye contact and an appropriate level of engagement.

22. Reports to work on time and maintains reliable and regular attendance.

23. Models the agency’s mission and core values in all communication and correspondence.

24. Communicates effectively in a culturally competent manner with a diverse member population and promotes favorable interaction with managers, co-workers and others.

25. Provides caregivers or other support system with information about the agency, process and procedures, and the various roles to assist them.

26. Use their voice to direct treatment.

27. Participates in continuous learning process as it relates to engaging consumers.

28. Required to meet applicable productivity and documentation requirements.

29. Assists with the design and implementation of internal and external trainings about the caregiver/support system.

30. Holds staff accountable to service delivery principles related to the consumer/family voice, choice, access, pace and process, through sharing personal experience, engaging in team meetings and consultations with staff internally and externally.

31. Models and coaches effective coping techniques for consumers, caregivers/support

system.

1. Practices self-care, remains aware that others may be contending with stress, and treats other with grace.

2. Performs other related responsibilities, as assigned, to support specific department/business needs.

Qualifications

Education Requirements

• High school diploma or GED preferred but is not required.

• Bilingual in Spanish required 

Experience Requirements

• Completion of Mental Health Worker Training Course is highly desirable.

• Position is reserved for an individual who has lived or knows someone with lived experience with mental illness or the mental health system.

Highly Desirable Qualifications

• Self-identified as a person with lived (or family) experience with mental illness and the mental health system.

• Ability to identify recovery principles and explain their recovery process.

Other Specific Requirements

• Must pass Department of Justice (DOJ), Federal Bureau of Investigations (FBI), and Child Abuse Index Check (CAIC) background clearance.

• Valid California Driver's license with two years of experience driving and clean driving record may be required.
