Job Description:

Position Summary: Provides outpatient nursing services to clients/patients in office and community-based setting; develops treatment plans with clients; provides health education; collaborates with primary care providers, treatment team, and community providers; assists with participants' access to medical care, housing, and other ancillary social support services. General knowledge base in Mental Health and Addictions nursing and Psychotropic medications.
Minimum Qualifications: Graduate of an approved school or college of nursing; current Florida licensure as a licensed practical nurse. Minimum of one year experience as an LPN in an acute care setting. Valid Florida driver's license with a good driving record.
Preferred Qualification: Mental Health Nursing experience with commitment to supporting recovery and independent skills of target population.
Special Skills, Knowledge and Abilities: Possesses written and verbal skills for effective communication. Performs basic EMR (Electronic Medical Record) computer skills. Detailed oriented with effective problem solving skills. Possesses leadership skills effectively leading a team. Knowledge base in Psychotropic Medications.
Essential Functions/Job Duties:
Performs Nursing Assessments and monitoring of clients/patients.
Obtains and enters orders into EMR from Attending/on-call physician
Administers medications and treatments efficient and effectively as prescribed by a physician.
Evaluates clients/patients nursing needs; develops and maintains Treatment Plan.
Coordinate with Case Managers inpatient care episodes to ensure continuity of care through transition to outpatient status.
Actively participates in PI (Peer Review) and disseminates information to staff.
Provide direct clinical and medical services to clients on an individual, group, family basis in office and community settings.
Provide on-call crisis intervention services during nighttime hours

Work Environment/Physical Demands: LPN's complete 80 % of work is in the field in different environments, mostly client homes. Staff can be exposed to various health conditions. Sits, stands, bends, lifts and moves intermittently during work hours. Is subject to frequent interruptions. Is involved with clients, residents, family members, personnel, visitors, government agencies/personnel, etc., in home and behavioral health settings. The job can be very challenging. The employee may be required to lift/pull/push/move up to 50 pounds. Must utilize conversational speech for effective verbal communication.
Position Type, Expected Hours of Work and Travel: This is full time position. This position requires flexibility in work hours to include some evening and weekend hours as needed to meet the needs of the clients. Frequent local travel is required. On call rotation is required.
