The Montefiore Clinical Transplant Coordinator (Practitioner) maintains standards of the practice for nursing as defined by the American Nurses Association Code of Ethics, Nursing Scope and Standards of Practice, and Nursing's Social Policy Statement. The Clinical Transplant Coordinator (Practitioner) actualizes the vision, mission, values, and balanced scorecard performance measures for Montefiore Medical Center. The Clinical Transplant Coordinator (Practitioner) participates in performance improvement/research activities; maintains professional practice standards and clinical expertise; and demonstrates leadership skills. The Clinical Transplant Coordinator (Practitioner) demonstrates core and unit-specific competencies based on the patient population served. Reporting to the Administrative Director, Transplant Services, this position, in collaboration with physicians and other health care providers, will be responsible for the planning, evaluation, coordination, implementation, and facilitation of all facets of care for patients and families undergoing the process of organ transplantation. Demonstrates advanced and diverse clinical knowledge and skills in the capacity of the clinician, educator, researcher, collaborator, and patient advocate to provide both direct and indirect patient care in a semi-autonomous, self-directed role in accordance with professional practice standards and clinical expertise. Coordinators who are physicians Assistants will, in addition, be responsible to their supervising physicians for clinical matters as required by state and hospital regulations.

Requirements:

NYS RN, NP or PA license required.

Coordinates the medical and social needs of transplant patients.

Collaborates with the interdisciplinary team in screening potential donors and recipients and developing and implementing treatment and discharge plans.

Promotes and develops organ donor programs. Bilingual in Spanish preferred.
