Job Description:

SUMMARY

 A Pharmacy Technician assists the pharmacist with prescription fulfillment, adherence counseling, coordination of care as well as providing safe and appropriate health and wellness coaching with patients in accordance with the policies and procedures of Barnes Healthcare, the state’s professional standards, and other regulatory entities. The Pharmacy Technician / Health Coach works under the direct supervision of a licensed pharmacist and will coordinate care with internal and external partners. A Pharmacy Technician / Health Coach will be working in all pharmacy roles and take a lead role in the coordination of patient pharmacy care of chronic conditions as well as health and wellness care.

PRIMARY RESPONSIBILITIES


  • Performs assigned tasks in a correct and timely fashion under the supervision of a pharmacist.

  • Assists the pharmacist with the preparation and dispensing of medications and supplies using appropriate techniques and following Barnes Healthcare policies and procedures.

  • Develop a customized care plan for employees, patients, and family members including goals and plan of action (Ecare Planning)

  • Coordinate Care with other health-related vendors for optimal care

  • Will be responsible for contacting employees, patients, and family members to provide necessary wellness coaching to reduce or eliminate high-risk behaviors.

  • Will be responsible for developing a wellness coaching relationship with Patients and Employees along with assisting them through the process of actively working towards better health by providing support, encouragement, and education.

  • Will provide continuous evaluation of the effectiveness of treatment through ongoing assessments of patient

  • Effectively communicates with facility personnel, patients, and family, as well as the operations teams to provide assessment benchmarks, coordinate care, the intervention of therapy to help facilitate overall health and wellness improvements.

ADDITIONAL RESPONSIBILITIES


  • Participate in on-call responsibilities of the pharmacy department as required.

  • Participate in Pharmacy Performance Improvement activities as required.

KNOWLEDGE AND SKILL REQUIREMENTS


  • Knowledge of medications and basic chronic care disease states     

  • Knowledge of basic pharmaceutical calculations       

  • Good communication skills, flexibility, and ability to work as part of a multi-disciplinary team.

  • Self-directed with excellent time management skills and the ability to work independently.

  • Ability to work under stressful conditions and under time constraints
