**Overview**

REACH Kidney Care, a division of Dialysis Clinic, Inc., is a kidney health management program designed to benefit patients along the continuum of kidney disease through health education, self-management, behavior change counseling, coordination of care with other providers, and patient navigation services. This program includes assisting patients with advanced chronic kidney disease (CKD) and end-stage renal disease (ESRD). The goal of the REACH care team is to educate, support, and empower self-care to improve kidney health and delay the need for renal replacement therapies (RRT). For people who do progress to ESRD, the REACH care team aims to increase preemptive renal transplants (before the need for RRT), facilitate a successful transition to RRT or medical management (MM), increase renal transplant rates after the start of dialysis, and address advance care planning.

Our REACH program has an exciting position available for a Registered Nurse with renal experience to work with patients who have Chronic Kidney Disease. The **REACH Chronic Kidney Disease Nurse Care Manager (RN)** provides education, care coordination and navigation services for our patients who are enrolled in our program.

**Responsibilities**

+ Conducts an initial assessment and an annual assessment thereafter, unless an assessment is warranted sooner due to a change in status or sentinel event

+ Creates and maintains an individualized plan of care

+ Educates the patient on kidney disease and other factors impacting his or her care such as comorbidities, including diabetes and hypertension, physical challenges, lifestyle factors and psychosocial factors

+ Engages the patient in his or her care utilizing evidence-based tools and guidelines

+ Helps the patient navigate and access appropriate care

+ Coordinates care with other providers also caring for the patient

**Qualifications**

+ A degree from an accredited Registered Nursing Program

+ A Registered nurse with current license

+ A minimum of 2 years of experience in one of the following areas is required:

+ Chronic Kidney Disease Management, Dialysis, or Renal Transplants.

+ Care Management experience a plus but not necessary

+ Must be technologically proficient with the ability to learn computer applications, medical platforms and virtual engagement tools.

+ Excellent interpersonal skills to build and maintain partnerships with patients, physicians and health care teams

+ Self-motivated with attention to detail and good time management awareness

+ Some occasional travel is expected - mileage reimbursement and per diem pay is offered
