Job Description:

JOB SUMMARY

The Home Visiting Outreach and Intake Coordinator will be joining a team of Intake and Outreach Coordinators at the Philadelphia Department of Public Health (PDPH), Division of Maternal, Children and Family Health (MCFH) to connect families living in Philadelphia to Philly Families CAN a centralized intake system for maternal and early childhood home visiting services. Home visiting programs are voluntary home-based services that offer families care coordination and connection to community resources, provide education on positive parenting, and promote child development and school readiness. Philly Families CAN is a service for families to learn more about and get connected home visiting programs in Philadelphia. Philly Families CAN consists of both a phone number and a website (PhillyFamiliesCAN.com) through which families and referring providers can access services. A Home Visiting Outreach and Intake Coordinator is responsible for receiving referrals from providers, performing outreach to pregnant and parenting families, and conducting an intake assessment to connect families to a home visiting organization that meets their goals. The Home Visiting Outreach and Intake Coordinator will report to Special Projects Manager.

This position is remote/socially distanced during this time. The Home Visiting Outreach and Intake Coordinator must live in the Greater Philadelphia area.

JOB SPECIFICATIONS


  • Conduct virtual and in-person outreach to pregnant and parenting families and to groups that serve families

  • Form and maintain relationships with staff at hospitals, health centers, schools and other community-based organizations serving pregnant women; provide presentations to providers about Philly Families CAN

  • Answer incoming calls from families and providers who call Philly Families CAN

  • Screen families requesting services using an intake questionnaire

  • Guide families through a shared decision-making process to select the home visiting program that best suites the family’s needs

  • Provide families with referrals for any additional support services to meet any immediate needs using Aunt Bertha online directory for social service organizations

  • Return calls of families and providers who complete the online web form on PhillyFamiliesCAN.com

  • Document all interactions with families and providers in the electronic case management system

  • Maintain strong working relationships with all home visiting providers participating with Philly Families CAN to ensure a thorough and up-to-date understanding of services provided

  • Document opportunities for intake process improvement and collaborate with Intake and Outreach Coordinator team, providers and MCFH Systems Administrator to update enrollment processes

  • Collaborate with MCFH Communications Coordinator on messaging for providers and families

  • Review data from electronic management system with MCFH Program Evaluation and Surveillance Manager.

  • Collaborate, coordinate calendars, and share best-practices/lessons-learned with Home Visiting Outreach and Intake Coordinator(s)

  • Attend Centralized Intake Steering Committee meetings

  • Other duties as assigned

Education


  • Minimum: Bachelor of Arts. RN or Masters in Social Work or a related field preferred

  • A strong history of relevant work experience may be substituted for educational requirements

Skills/Experience


  • At least 1-2 years of work experience linking families to essential services. Experience as a home visitor, case manager, or community health worker preferred

  • Genuine interest in maternal and child health issues in Philadelphia

  • Multi-lingual, preferably Spanish

  • Comfort with or interested in using language interpreter services

  • Exceptional verbal and interpersonal communication skills

  • Experience in communicating with families and providers on the phone

  • An understanding of trauma; experience and training in providing trauma-informed care

  • Experience or knowledge of Philadelphia healthcare and social service sectors

  • Proficiency multitasking: recording enrollment information while giving sincere attention and authentically engaging families/callers

  • Strong data entry and computer skills; proficient in Microsoft Windows, Word, Excel, and Outlook

  • Experience with digital enrollment software or ability to learn new technological tools swiftly and confidently

  • Experience working with ethnically, culturally, and sexually diverse individuals, communities, agencies, and organizations

POSITION TYPE AND WORK SCHEDULE

Full time position, preferred hours are Monday through Friday 10:00 am to 6:00 pm.  

SALARY AND BENEFITS

Salary range is between $40,000-$50,000 and is commensurate with experience and qualifications.

WORK ENVIRONMENT

Standard office setting with extended periods at workstation and periodic use of office equipment.

TRAVEL

Local travel to multiple sites and community meetings, as needed.

PHYSICAL DEMANDS

Ability to travel to external meetings, transport materials, routinely moves equipment or supplies weighing up to 15 lbs.
