Kendal at Lexington is currently recruiting for a Resident Services Coordinator/Social Worker for our Borden Nursing Center.  This position will be responsible for ensuring that the medically related emotional and social needs of the residents are met and maintained on an individual basis in accordance with Federal, State and Kendal standards, guidelines and regulations.  The selectee will be responsible for developing comprehensive assessments of residents; assisting residents and their families cope with immediate effect of the decision to move to the facility; orient families to the facility; attend care plan meetings; complete mandated documentation in a quality and timely manner;  mediate issues; provide link to the appropriate community resources and make appropriate referrals; develop comprehensive discharge plans; educate about Resident Bill of Rights and Resident Abuse regulations; participate in policy development and participate as a member of the social services work team.

Qualifications include:  Ability to make effective decisions.  Ability to collaborate and communicate in a most effective way.  Must possess above average communication skills (both orally and written).  Must be results driven, thorough and flexible.  Must be motivated to succeed in a team environment.  Must possess a Bachelor’s Degree in Social Work from an accredited college of social work.   Must be able to work with and relate to the disabled, elderly, emotionally upset and at times hostile people with our facility.
