The Senior HIM Associate is responsible for performing various types of health information review and analysis. Provides basic clerical support to the department which includes the retrieval, chart tracking, scanning and quality control functions of medical record chart processing as well as analyzing medical records for documentation of physician deficiencies. Maintains data in management systems. Responsible for identifying duplicate medical record numbers and forwarding the information to EPIC Patient Identity Management. Responsible for completing document corrections in EPIC system.

Responsibilities

A day in the life of a Senior HIM Associate at Hackensack Meridian Health includes:

Accurately analyzes medical records for document deficiencies while meeting established accuracy and productivity standards.
- Reviews and works all assigned queues.
- Follows up on missing information and transcribed reports in order to facilitate the chart completion process.
- Collects information to analyze, verify and generate data for regulatory compliance as needed.

Coordinates study/audit activities.

Receives all requests/audits of medical records from internal/external entities. Accurately and timely coordinates retrieval of records for audit studies, research, registries, etc.

Schedules dates, time and location for upcoming audits. Enters charts into Release to Inspector work queues. Provides summary of audit activities to appropriate HIM leader.

Retrieves medical records in a timely manner to facilitate patient care for hospital operations.

Transports and retrieves medical records and other related material to and from other departments appropriately while maintaining patient confidentiality.

Checks to ensure all discharged charts have been received.

Prepares medical records for scanning with a focus on accuracy and productivity standards.

Checks chart information and preps chart for scanning, utilizes chart tracking to track chart location.

Responsible for Quality Control of HIM scanned documents and integrated scanned documents.

Reviews each document to ensure that documents are appropriately placed in chart and that information is not duplicated.

Responsible for forwarding of charts in the computer system to the Analysis Needed Work Queue.

Responsible for the scanning of any loose documents received by the department.

Accurately facilitates the Medical Staff chart completion process through active communication with physicians and other hospital staff. Assists outside physician practices with correspondence.

Maintenance of Equipment and Supplies: Operates office equipment properly and makes recommendations for maintenance, replacement and purchase supplies.

Participates in training, in-service and meetings:

Assists in orientation and training of team members/students upon request. Consistently evaluates departmental processes and makes suggestions for improvement.

Responsible for computer literacy: Functional knowledge of all HIM and other related information systems required to perform daily tasks.

Adheres to HIPAA guidelines.
-
Other duties and/or projects as assigned.

Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

Education, Knowledge, Skills and Abilities Required:
- High School Diploma or Educational Equivalent.
- Good organizational skills, with orientation to detail and accuracy.
- Good written and verbal communication; including the ability to work interdepartmentally.
- Ability to work independently.
- Good computer skills.
- Experience working with standard office equipment: telephone, facsimile, copier, printer and scanner.
Education, Knowledge, Skills and Abilities Preferred:
- Knowledge of EPIC and Google Chrome applications a plus.
- Knowledge of Medical Terminology.
- Experience in health care or related field.
