Summary
The LMSW Social Work Coordinator, in collaboration with Texas Health Resources-Harris Methodist, will spend 50% of their time to oversee a follow up parental compliance/adherence program for identified high risk infants and children post discharge from THR Harris. The LMSW Coordinator will work in collaboration with the families and medical team to establish a guiding and non-threatening environment to increase appropriate medical follow-up for infants and children. This position will be responsible for the development, coordination and implementation of a post discharge program to assess parental compliance with referral/ follow up appointments for an identified at risk population of infants and children. The remaining 50% of the position will follow the Coordination of Care (CACO) job description. The LMSW Coordinator will identify appropriate interventions to encourage and support parental compliance when failure to follow post discharge medical plan of care has been found. High-risk conditions could include but are not limited to the following: VP shunt, retinopathy of prematurity (ROP), metabolically fragile diagnosis, live dysfunction, uncontrolled diabetes ( Hyperinsulinism), cardiac diagnosis, respiratory diagnosis, history of maternal drug use, child protective services history or teen mother.

Qualifications

- MSW or MSSW from an accredited school

- Current Texas License for LMSW or LCSW is required

- Basic Life Support Certification is required within 30 days of employment
