Job Description:
CENTERSTONE POSITION DESCRIPTION
Position Title: Case Manager-Community Action Treatment (CAT) Team-Manatee County
Position Summary: Will assess, plan, develop, coordinate and provide case management and supportive services for children ages 11 -21 with mental health and/or co-occurring disorder as a part of the enhanced Children's Community Action Treatment Team which serves clients and their families in Manatee County.
Minimum Qualifications: Bachelor's degree in social work, psychology or related field. At least one year related experience working with children and their families. Valid Florida driver's license with a good driving record.
Special Skills, Knowledge and Abilities: Knowledge and understanding of the characteristics of children with mental health issues, their families, and family systems. Ability to work well with community partners and in a team environment is essential. Services are provided in the community.Highly motivated, self-directed, willing to work a variety of hours, including evening, some weekends, and on-call.

Essential Functions:
- Provide case management services to an assigned group of clients. Assumes responsibility for assessing child and family strengths, challenges and needs.
- In conjunction with the team, responsible for developing, writing, implementing, evaluating and revising overall treatment goals and plans per on-going assessment of needs and input from the child and family.
- Conduct on-going assessment of behavior, mental status, signs and symptoms of mental health disorder, physical and dental health, substance use, and educational, social, developmental and family functioning.
- Refer child and family for additional services as identified.
- Act as liaison and consult with the school system, community agencies and families to maintain coordination of the treatment process. Advocate for child and family's rights and needs.
- Provide support to child and family as a role model teaching life skills, positive discipline, parenting skills, financial responsibility and homemaking skills.
- Assist families with developing natural support systems.
- Provide on-call crisis intervention services during nighttime and weekend hours as assigned.
- Provide transportation, respite and tutoring services as assigned.
- Performs other related duties as assigned by supervisor.

Work Environment/Physical Demands: Services are conducted primarily in the community, spends time in outpatient behavioral health settings and operates in a professional office environment; can be exposed to various health conditions. While performing the duties of this position, the employee is regularly required to observe treatment environments and to use speech and hearing to interact with others in person, via telephone, using computer technology and in writing. Reasonable accommodations will be made as needed.

Position Type, Expected Hours of Work and Travel: This is a full time, hourly position. Routine days and hours of work are Monday through Friday from 8:30am to 5:00pm. Some evening and on-call hours required to meet client needs.
