Program Coordinator, Adult Education- Diabetes Empowerment at Catholic Charities

The Program Coordinator, Diabetes Empowerment is a bilingual (Spanish / English) position responsible for supervision and management of the Spanish Diabetes Self-Management Program at Esperanza Center. They will serve as the primary point of contact with the Grant Officer at the Maryland Office for Minority Health and Health Disparities.


This position is grant funded through June 30, 2021 with the possibility of renewal on an annual basis. 


Program Coordinator, Diabetes Empowerment is responsible for:



  • Provide management and oversight for the implementation of an evidence-based program curriculum, participant recruitment, data collection and management, and program assessment.

  • Collaborate with and assist Esperanza Health Services Staff to recruit participants and manage data collection

  • Co-Facilitate program sessions per the designated, evidence-based program curriculum and using principles of adult education; sessions will take place once per week across 6-week sessions

  • Coordinate program scheduling with community facilitators, participants, and volunteers

  • Prepares regular data and narrative reports per the Minority Outreach and Technical Assistance (MOTA) grant agreement

  • Represent the Center with professionalism in all interactions with staff, clients, community partners, and volunteers

  • Prepares and processes invoices, check requests, field expense vouchers and other documents related to program expenses (travel, food, material purchases, other as appropriate)

  • Processes appropriate accounting and IT forms and helps to resolve issues that may arise in a timely fashion.

  • Responsible for managing program facilitators (1-3) and coordinating the work of others

This position requires:



  • Associate's Degree, or the combination of two years of education and experience in Human Services, Administration, Volunteer Management, Emergency Response, or a related field.


  • Two years of experience in Education, Human Services, Volunteer Management, Public Health, Spanish, or a related field.


  • Complete requisite facilitator training approved by the Self-Management Resource Center within 45 days of start, or as soon as considered safe to participate due to the COVID-19 pandemic.

  • Ability to learn and master database software.  AbilityRequires the ability to utilize computer systems and software necessary to perform position functions. Basic Windows PC, web browsing (i.e. Chrome, Internet Explorer, etc.), and Microsoft Outlook skills required. Other Microsoft Office application knowledge, such as Word, Excel PowerPoint, Skype, and OneDrive desired.


  • Bilingual (Spanish/English).

  • Must have reliable transportation to visit multiple sites. If driving on Agency business, must be at least 21 years of age and have valid driver’s license issued by the state of residence, with no more than 3 points.
