JOB OVERVIEW
Reviews and analyzes medical records to ensure quality and compliance with medical record standards, including release of information. Responsible for generating data, completing and submitting reports in a timely manner to appropriate sources. Responsible for accurately maintaining provider data base and associated processes. Maintains strict security and confidentiality of patient information. Serves as backup for medical records assistant. Responsible for quality service delivery and internal/external customer relations for the department and Madonna as a whole, including the mission and values for the department and facility.

ESSENTIAL FUNCTIONS


  1. Assists with preparation of medical records for review by others including; supervisors, managers and committees, employees conducting quality assurance studies and research, Quality Improvement Organization, external accrediting/licensing agencies (NDHHS, CARF, The Joint Commission, etc.), external financial auditing organizations and other request.
  2. Appropriately collects data from multiple sources to complete files and reports for internal and external sources.
  3. Accurately prepares and types reports on a personal computer in designated format and in assigned time frames.
  4. Performs audits of medical records to assure accuracy and completeness.
  5. Maintains working knowledge of state and federal regulations related to medical records.
  6. Discloses protected health information appropriately, when requested.
  7. Files medical records in appropriate filing area (incomplete, outpatient, or permanent files).
  8. Coordinates off-site record storage process, including preparing medical records for off-site storage, maintaining an inventory of records stored off-site, and ensuring that destruction is according to state standards.

A. Communicates with appropriate individuals regarding the results of documentation and quality reviews; must be diplomatic and persuasive in presenting the expectation of excellent patient record documentation for compliance, customer satisfaction and revenue generation.
B. Provides access to medical records to authorized individuals.
C. Answers questions about the medical record content or patient stays at Madonna to authorized internal and external customers.
D. Communicates with providers to schedule system training.
E. Communicates with and advise Manager of situations which may impact department and facility.
F. Takes instruction from Manager to perform other functions as assigned in order to achieve the goals within the department and or system.

QUALIFICATIONS (Education/training and/or Experience)

EDUCATION:
High school diploma required. Advanced education in HIM or
other applied health or business-related field preferred.

EXPERIENCE:
Requires two years relevant working experience in one of or a
combination of the following: Health care data management, medical records maintenance/review/analysis and/or Medical Assistant.

SKILLS: Requires excellent organizational skills and attention to detail. Requires effective communication skills and telephone etiquette. Requires knowledge of medical records and processes. Knowledge of state and federal regulations related to medical records administration. General computer skills with specific proficiency in Microsoft Office applications, such as Excel , Outlook and Word .
