SCHEDULE: Full-time

10 HOUR DAY SHIFT - SATURDAY, SUNDAY, MONDAY, TUESDAY SCHEDULE

The RN Case Manager provides hospital case management/utilization review and discharge planning to assure that the patient progresses through the continuum of care, and is discharged to the least restrictive environment.

Palms of Pasadena is located in a beautiful Pinellas County beach community, Palms has been serving residents and visitors for more than 50 years.

This 307-bed facility is Joint Commission accredited and committed to providing high-quality medical care in a friendly hospital environment, offering a full range of services including emergency, surgical services, orthopedics, cancer, surgical weight management at the Florida Bariatrics Centers, wound care, a Rehabilitation Center and Sleep Disorders Center. The hospital also recently opened the area's first Acute Care for the Elderly (ACE) unit.

QUALIFICATIONS

Licensure/Education:

Current State of Florida RN licensure

Completion of an RN Diploma program or related healthcare program

National Case Management Certification strongly desire

Qualifications:

Minimum of three (3) years recent experience in an acute-care hospital setting required

Case Management experience is preferred
