The Contra Costa Youth Continuum of Services (CCYCS) program is a public/private partnership with Contra Costa Health Services’ Health, Housing, and Homeless Program and Heluna Health. The Contra Costa Youth Continuum of Services (CCYCS) is a program of Contra Costa Health Services that provides outreach, interim shelter, transitional housing, and permanent housing for homeless and at-risk youth ages 18 to 24. The guiding philosophy of CCYCS services is that every youth has strengths and the ability to make positive changes in their lives. Salary: $54,080 - $61,500, Annually

ESSENTIAL FUNCTIONS


  • Responsible for general oversight of all case management services within the Contra Costa Youth Continuum of Services Program (West or Central).

  • Develops relationships with local housing agencies, property managers, and landlords to create and identify housing options and opportunities for Contra Costa Youth Continuum of Services consumers.

  • Trains and supervises team of case management staff and employment and education specialist.

  • Facilitates weekly case management meetings.

  • Responsible for case management staff scheduling and assignments.

  • Supports Case Management staff with caseload of 5-10 high-needs and severely vulnerable clients as needed.

  • Conducts audits of client data and file documentation.

  • Works closely with other homeless services, vocational institutions, drug and alcohol abuse treatment programs, and mental and medical professionals to develop effective program.

  • Attends Continuum of Care Board and other community meetings.

  • Participates in weekly resident meetings.

  • Responsible for assuming the duties of the CCYCS Program Manager if needed.

  • Participate in an on-call rotation to respond to after-hours crisis.

  • Ability to work with diverse populations.

  • Ability to communicate clearly, both in verbal and written forms.

  • Ability to be supportive and provide guidance in a non-judgmental manner.

  • Other duties as assigned.

JOB QUALIFICATIONS


  • Be fingerprinted and pass a criminal background check.

  • Trained in CPR (agency will train).

Desirable Qualifications


  • Experience working in Contra Costa County and knowledge of social service providers in the area.

  • Background in adolescent mental health services and/or homeless services.

  • Bi-lingual in Spanish; bi-cultural.

Education/Experience


  • Bachelor’s Degree from an accredited college or university in Social Work, Psychology, Health Science, or closely related field.

  • 3 years of case management experience working with homeless consumers.

  •  Five (5) years of full time, or its equivalent, experience working in a program serving homeless persons, 3 years of which need to be in the role of case manager

  • 50 semester units or 90 quarter units of college coursework.
