Southwestern Health Resources (SWHR CIN) seeks to hire a Patient Navigator to work Full Time supporting the Referral Management department.

Salary range is Min $15.70 - Max $23.17 - based on relevant experience

Monday-Friday 8am-5pm
Training schedule is Monday-Friday 8am-5pm for 6 weeks
Job Description
The Patient Navigator is a critical role in ensuring that individuals in our community are provided with every opportunity to have all the necessary care required for themselves and their family. These individuals make a difference by guiding patients through the complex medical system.
Process and complete referrals as directed by Primary Care Providers, following Patient Navigation protocols and expectations.
Utilizes exceptional customer service skills to communicate with patient, referring and service providers, and clinic staff to complete referrals accurately and expediently to minimize errors and reduce delays in care.
Documents referral information in tracking program to be consistent with documentation in the EMR.
Communicates appropriately with all stakeholders.
Messaging primary care providers or designated clinic staff any and all patient care related concerns or relevant information.
Redirecting clinical conversations to clinical personnel.
Utilizing Patient Navigation team for assistance to complete referrals by utilizing appropriate communication forms and templates.
Submits documentation to obtain insurance referral authorizations and pre-certification for ancillary services, diagnostic testing and imaging.
Coordinates pre-admission testing requirements with clinic personnel and patient.
May educate patient on expectations.
Follows up on set tasks and pending or incomplete referrals.
The ideal candidate will possess the following qualifications:
Education
High School Diploma or Equivalent Required
Experience
6 months Scheduling or Health Care Office Experience Required
Referral Processing or Scheduling Experience Preferred
Demonstrated knowledge of Intermediate Computer Skills Preferred
