Job Description:

Community Solutions Inc. (CSI) is seeking a Community Transition Coordinator for our program in Cranston, Rhode Island on an as needed basis. You will be a part of a team helping to guide clients in their development and assist them to become a positive influence in the community. Responsible for all aspects of client discharge planning. Provides exit packets, community based referrals with follow up, collects and reports data to DOC, participates in reentry forums, and is expected to demonstrate seamless integration with DOC data entry system. Daily communication with DOC staff is required.

Community Solutions Inc. (CSI) is a nonprofit organization that promotes the responsible citizenship, accountability, and well-being of individuals and families involved, or at risk of involvement, in the child welfare, juvenile justice, or criminal justice system. Since CSI’s founding in 1962, it has been a pioneer in the community-based service. We have been delivering supervision, treatment, and comprehensive individualized services to help clients interact more effectively in their communities and become productive citizens. Using this plan we built the first ever halfway house in New England which has now been replicated and grown to almost 50 programs in 9 states. For over 50 years, CSI has been building upon its successful foundations and has been expanding its services to serve thousands of adults, youth, and families each year.

Essential Functions include but are not limited to:

  • Maintain all aspects of client discharge planning.

  • Providing and completing client exit packets from the Department of Corrections

  • Coordinating all exit requirements with Probation and Parole Officers

  • Coordinating with Prisoner Reentry Councils and Forums

  • Constructing Discharge plans

  • Referring offenders to community based services

  • Following up to ensure follow through with referrals

  • Collecting and reporting data on services and performance metrics

  • Collaborating with RIDOC in regular performance management meetings

  • Collaborating with internal CTC team for weekly case review and supervision

  • Conducting Post Release Follow Up

  • Reporting Post Release Data

  • Completes client paperwork via RIDOC Case Management electronic system

  • Responds to client and program crises and reports incidents to supervisor.

  • Attends and participates in staff meetings, supervision, and on-going agency training

  • Bachelor’s degree in a social science field and one year experience in social services or similar field. Education may be substituted with experience on a year for year basis.

  • Excellent oral, written and interpersonal skills desired.

  • Experience with Microsoft Office.

  • Valid driver’s license and transportation
