As an expert clinician, THE NURSE NAVIGATOR is responsible for coordination / facilitation of adult complex hematologic malignancy patient care and services across inpatient and outpatient settings from first consult through long term follow-up. S/he collaborates with the multi-disciplinary team to assure an organized approach to delivery of care. S/he acts as a liaison with physicians, internal / external departments and other agencies related to creation and follow through of the patient's individualized treatment plan. S/he communicates with responsible parties about any scheduled or unplanned deviations from the plan of care and works to resolve /remove obstacles to access or delivery of recommended therapies

LICENSURE/CERTIFICATION/REGISTRATION: Current American Heart Association CPR certification. ONS Chemotherapy Certification within 6 months and OCN within 3 years of hire. Valid LA RN License

EDUCATION: BSN required.

EXPERIENCE: Minimum 3 years experience in heme-malignancy setting

SPECIAL QUALIFICATIONS: Excellent verbal and written communication skills. Use of databases and IT collaboration tools highly desired or needed to acquire within 3 months after hire

HOURS OF WORK: M-F Clinic hours with minimal after hours and weekends
