Job Description:

Family Resources is looking for a passionate, organized, and professional Registered Nurse Care Coordinator. The Integrated Health Home (IHH) Registered Nurse Care Coordinator will interview and guide clients and their families with behavioral/mental health conditions as a member of the IHH Team. Provide family-driven and community-based, comprehensive, and culturally competent care in the provision of services. Provide health education to individual members and families and develop health education programs for population health management. Responsible for IHH care coordination of FRI residential clients. Responsible for coordinating all medical and psychiatric appointments for all foster group care clients.

ESSENTIAL FUNCTIONS


  • Provide planning, care, and coordination functions, including follow-up contacts with family and community partners.

  • Coordinate and triage referrals, working with referral sources and provider agencies, providing consultative services.

  • Interview clients and their caregivers to obtain and interpret social history information and/or problems and barriers.

  • Provide education, monitoring, and support, with follow up to children and their families, as well as community partners.

  • Develop and implement multidisciplinary care plans, based on the principles of family-driven care.

  • Evaluate and factor in the child’s condition, age, psychological, educational, and socio-economic conditions, producing measurable goals and objectives in partnership with the family and client.

  • Integrate contributions of the child and family into the multidisciplinary care plan.

  • Provide education to members and families regarding a healthy lifestyle, medications, health conditions, etc. as needed.

  • Evaluate and document the effectiveness of the IHH care plan.

  • Coordinate with Access/Intake staff and residential program staff to admit residential clients to IHH.

  • Attend weekly Clinical Review Meetings and all staffings for residential clients.

  • Responsible for scheduling and tracking medical, dental and mental health appointments for all residential clients and relaying this information to the appropriate residential program staff.

  • Coordinate all psychiatric services for foster group care members. Serve as liaison to psychiatrist/mental health provider.

  • Attend/transport clients to all psychiatric appointments to ensure appropriate communication between provider and residential program. Attend/transport to medical appointments that require a higher level of medical knowledge.

  • Provide follow-up education, nursing care, etc. to residential clients and staff after appointments and relay all information to caseworkers.

  • Document all services provided.

  • Serve as liaison with community providers by providing expert knowledge of the service system.

  • Provide On-Call telephone consultation on a rotating basis, including meeting with clients after hours for crisis management.

  • Facilitate communication between the families and care providers, serving as an advocate for the child and family.

  • Assist the family to obtain necessary knowledge of health care financing entities such as Medicaid, EPSDT, SSI, state waivers, Iowa Cares, and the hawk-I program.

Requirements: Active Iowa Nursing License required.

Must have current proof of car insurance, able to pass driving background check and have reliable transportation.
