Provides clinical services including behavioral health assessments, crisis assessments, individual counseling and family counseling through telehealth from our virtual telehealth hub, during extended hours including evenings and weekends.
Completes timely and compliant clinical documentation and billing practices following all HRBH policies and standards set forth by HRBH policy, including but not limited to collaborative documentation, documentation signed with 24 hours, ensuring discharges are completed, etc.
Meets a productivity standard as this position is a Pay for Performance position, which requires that staff who work 40 hours per week complete 50 hours of billable services per pay period. (Pro-rated for those who work less than 40 hrs per week)
Provides case management and referral information for individuals assessed via the telehealth hub to services within the individual's home county or within 5 miles of their home county as well as linkage to telehealth services should this be at the individual's preference.
Takes ownership for individual and referral source satisfaction by returning phone calls in a timely manner, communicating with referral sources with appropriate authorization, link and/or serve as advocate for individuals as needed.
Encourages individual responsibility and recovery by utilizing evidenced-based interventions, providing comprehensive and ongoing assessment, referring individuals to appropriate internal and external services, involving and educating family, seeking and utilizing supervision, developing individualized and comprehensive plan of care based on the individual's strengths, needs, abilities, and preferences.
