Position Overview
Responsible and accountable for evaluation, coordination and administration of quality occupational therapy services. The Occupational Therapist is expected to adhere to all Aveanna policies and procedures, Occupational Therapy standards of practice and Regulatory agency guidelines; support program growth through program development and implementation; meet productivity standards and contribute to a positive work environment that is compatible to achieving organizational goals.

Essential Job Functions
Conduct comprehensive diagnostic evaluations on children, adolescents and/or adults consistent with and appropriate for patient diagnosis.
Design plan of care utilizing functional, measurable and attainable goals, based upon interpretation of evaluation results and patient and/or family goals
Provide a written evaluation defining patient strengths and barriers related to age appropriate functional performance, goals and expected outcomes
Maintains plan of care by performing timely documentation of client performance, modifying goals and treatment as needed, and determine discharge care plans as appropriate.
Responsible for discharge planning, including collaborating and educating the patient and his/her caretakers on home exercise plan to follow discharge.
Participate in coordination of services (including discharge planning), with health care team, physicians, parent/guardians, nurses, and appropriate others
Documents in accordance with Occupational Therapy Standards of Practice, Aveanna policies and procedures and Regulatory Agency guidelines.Documents effectively resulting in no technical or clinical denials subsequent to review of documentation by payers

Maintain effective communication between staff, health care team members, and family.
Mentor peers, new graduates and serve as Clinical Instructor for Occupational therapy students as needed
Participate in program development, marketing, and continuing education for skill development and competency.  Attend and participate with in-services as indicated.
Supervises other personnel (COTA, HHA) when required
Demonstrate proper use and maintenance of department equipment and supplies
Follow policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws

Requirements
Occupational Therapy Master’s Degree from an accredited university/program
Current and unrestricted license in the state of practice
NPI number
Current CPR certification
Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures

Additional state specific requirements:
Any additional Occupational Therapist licensure board requirements in the state of practice
Continuing Education as required by state

Preferences
One (1) year of recent experience as an Occupational Therapist in an acute care rehabilitation setting
Pediatric experience

Must be able to sit and climb stairs
Must have visual and hearing acuity
Must have strong sense of smell and touch
Must be able to sufficiently reposition patients and move equipment without assistance
Must be able to appropriately respond physically and mentally to emergency situations in the home or outpatient setting

Environment
Must be able to function in a wide variety of environments which may involve exposure to pets, allergens and other various conditions
Possible exposure to blood, bodily fluids and infectious diseases
