Essential Duties:

40 % of job: Counseling Essential Duties:


  • Applies specialized clinical knowledge and advanced clinical skills in the areas of assessment, diagnosis, and treatment of mental, emotional, and behavioral disorders, conditions and addictions, including severe mental illness in adults.
  • Develops and maintains effective working relationships with individuals and families referred for counseling.
  • Responsible for provision of evidence based counseling services by using evidence based treatment modalities to provide psychotherapy (i.e. Motivational Interviewing, Cognitive Behavioral Therapy, Brief Problem Solving Therapy, etc.)
  • Uses the Diagnostic and Statistical Manual of Mental Disorders (DSM), the International Classification of Diseases (ICD), and other diagnostic classification systems in assessment, diagnosis and other activities.
  • Develops intervention strategies to meet client short-term and long-term treatment goals.
  • Makes appropriate referrals to available MHM Services, including case management and psychiatry.
  • Promotes interactive communication with other MHM programs and/or services, the community and other community agencies.
  • Responsible for day-to-day operation of case flow, assessments, and crisis interventions.
  • Implements policies and procedures and recommends any needed modifications.
  • Maintain productivity expectations established by supervisor.
  • Participates in activities to develop and maintain quality assurance indicators that evaluate the effectiveness of the counseling program.
  • Assists in the planning, coordinating and implementing of projects and programs to include but not limited to identifying needs, researching and managing caseloads.
  • Develops and maintains a cumulative record for each client receiving counseling services as required by professional licensure standards and MHM policies and procedures.
  • Performs other duties as assigned.
    60% of job: Consulting Essential Duties

  • Provide onsite consultation, education, brief assessment and evidence based interventions to address chronic disease and acute behavioral health needs in a primary care medical setting.

  • Provide an average of 8-10 brief Behavioral Health Consultations per day to include individual, couples, and/ or family consultations including:
  • Focused, functional, and strength-based assessment and diagnosis in the medical clinic;
  • Psychoeducation for patients and their support systems;
  • Provide consultant services to clinical staff based on Primary Care Behavioral Health (PCBH) Consultation model.
  • Uses evidence based modalities (ie. Motivational Interviewing) to develop behavioral strategies aimed at symptom reduction for primary care patients;
  • Self-Care Plan development and skills training to facilitate disease self-management, improved coping, distress tolerance, stress reduction, and relaxation; and address concerns such as

    • Brief problem solving, cognitive intervention aimed at modifying negative thinking and promoting self-efficacy;
    • Medication adherence counseling and disease self-management counseling;
    • Substance use/abuse evaluation, identification of maladaptive coping strategies, and development of harm reduction strategies.
    • Determine if patients have need for specialty mental health services.
    • Be immediately available in the event of a psychiatric emergency, evaluating crisis situations and applying appropriate interventions.
  • Work a flexible schedule which includes unscheduled patient visits, same day patient visits and scheduled patient visits referred by Providers, members of interdisciplinary team and self-referrals.

  • Conduct group classes and group visits to promote skill building, education and improved health for the patient.
  • Consult with clinical staff to enhance understanding of the patient, provide decision support for treatment planning and assist in the implementation and monitoring of biopsychosocial treatment plans.
  • Use the Diagnostic and Statistical Manual of Mental Disorders (DSM), the International Classification of Diseases (ICD), and other diagnostic classification systems in assessment, diagnosis and other activities as appropriate.
  • Assist with care coordination and access to resources for patients to the interdisciplinary team, as needed.
  • Assist in provider/clinical staff training and education in Behavioral Health Consultant model of care and integrated behavioral health.
  • Assist medical team in setting up systems to implement integrated behavioral-primary care and the Behavioral Health Consultant model of care.
  • Monitor team members to ensure effective execution of services.
  • Assist in the planning, coordinating and implementing of projects and programs as assigned by Behavioral Health Manager.
  • Develop linkages with Methodist Healthcare Ministries programs and community agencies as appropriate.
  • Uses data to assist in improved planning, development and effective use of resources.
  • Attend and participate in meetings and Quality Assurance activities as required.
  • Performs other duties as assigned.

Qualifications:

Education/Experience :

Master's degree in Social Work, Counseling, Psychology, Marriage and Family Therapy or related field; two years clinical experience providing counseling to include experience or knowledge of primary care settings operation.

Language Ability :

Ability to read, analyze, and interpret documents such as professional journals, operating manuals, maintenance instructions, technical procedures, or government regulations. Ability to read, analyze and interpret medical documents. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. Bilingual in English and Spanish required.

Reasoning Ability :

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions furnished in written, oral, diagram, or schedule form and deal with several abstract and concrete variables. Ability to perform independently.

Certificates and Licenses :

Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC) or Licensed Marriage and Family Therapist (LMFT).

Computer Skills :

Ability to demonstrate working knowledge of the Internet, Outlook, PowerPoint, Word, Excel and electronic health records software. Able to document efficiently in electronic health record.

Other:

Ability to drive and have access to a car, maintain a valid driver's license and auto liability insurance.

Frequent in-town travel required. Periodic regional travel required.

While performing the duties of this job, the team member is exposed to an office and residential environment having direct contact with patients and clients. Position necessitates bending, stooping, reaching, kneeling and crouching, sitting, standing and walking for periods of time. Must be able to lift and maneuver 25 pounds. Maybe required to work special events evenings and/or weekends.
