Description:





The primary purpose of your position is to provide plan, organize and direct the overall operations of the assisted living program in this community in accordance with current federal, state and local standards, guidelines, and regulations, our established policies and procedures, and as may be directed by the Administrator. The Team Lead is to supervise the day-to-dayactivities performed by the anam cara staff and has been delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.





ESSENTIAL FUNCTIONS




Resident Rights: All employees are required to maintain the confidentiality of resident care information and comply with all THSC HIPAA policy and procedures. The Anam Cara is to ensure all Residents are treated fairly, with kindness, dignity, and respect. Anam Cara's must ensure that all nursing care is provided in privacy and privacy is afforded to all Residents within the community. Anam Cara's will knock before entering the Resident's private bedroom. All Anam Cara's are required to report all allegations of resident abuse, neglect, and /or misappropriation of resident property. The Anam Cara should honor the Resident's request to refuse treatment and inform such requests to the nursing supervisor. The Anam Cara is required to report all complaints and grievances made by the Residents to the director of nursing.


Activities of Daily Living: The Anam Cara is responsible for providing all activities of daily living to the Residents living within the Community. These activities include, but are not limited to bathing, dressing, toileting, transferring residents, grooming and personal hygiene, meal services, clean up, feeding, serving, welcoming residents when moving in and assisting when moving out of the Community, responding to Residents needs, conducting range of motion, position, lifting, and turning, participate with rehabilitation and restorative needs, and conduct light housekeeping as needed including upkeep ofcommunity and resident rooms.


Communication: Provides regular and timely feedback with other members of the Community, the Residents within theCommunity and to the support staff within the community. Communication is honest, respectful, and reverent to Residents and at the Residents pace. Keeps Residents informed of the daily happenings within their community. Promotes an environment of good communication within the community. Ensures medical, spiritual, and social concerns are communicated to appropriate support staff.


Medication Administration: The Anam Cara willprepare andadministermedicationsas directed.Medicationswill be administered withinorderedtimeframesinaccordance withestablishedprocedures. TheAnam Cara willensure allmedications andtreatments are keptsecure,i.e,medication carts and treatment carts arelocked when not insight.


Skills and Knowledge: Each Anam Cara will maintain certification and is able to demonstrate competency of clinical skills as well as overall knowledge of care plans, profiles, and work assignments. Each staff member ensures work is conducted accurately and within specified timeframes for completion. Reports to others both in and outside ofCommunity all necessary information to ensure quality outcomes are achieved and maintained. Assists in the review of work assignments and schedules to update and change task objectives. Ensures tasks are performed in accordance with community policy and procedures and work routines.


Documentation: The Anam Cara is required to maintain appropriate documentation of Residents within theCommunity according to community policy and procedure. Ensure appropriate changes in Resident condition are documented appropriately and required staff members are informed. Each Anam Cara will participate in the continual updates of Resident Care Plans. Enters information and documentation accurately in Point of Care in accordance with company policy, practice and standards.




QUALIFICATIONS:




High School Diploma or equivalent.


Minimum of one-year experience within a healthcare setting preferred.


Previous experience working in long-term care or with the geriatric population preferred.


Must possess superior customer service skills and professionalism.


Must possess outstanding verbal and written communication skills.


Ability to adapt to ongoing change and work in a fast-paced, customer-driven environment.


Possesses interpersonal and relationship building skills to work in a cross-functional team as well as the ability to work with residents based on their cognitive ability.
