The Community Hospice has a wonderful opportunity for a FT RN on-call position to work as part of our Hospice Care Team caring for patients and families in the Columbia/Greene county region. This is a full-time On-Call Visit Nurse position. As an RN you will provide care to patients in their residences in collaboration with our Hospice team after hours and on every other weekend. A majority of the hours are evenings and nights during the week from 4pm-8am. This does require an every other weekend commitment with 24 hour coverage. The On-Call nurse will never be bored as they will help those in need with medication, supply deliveries or more urgent matters like a patient in need of getting their end of life symptoms managed. Finally, they will demonstrate SPHP's Standards of Behavior and Core Values.
The Hospice OnCall RN's are responsible for:
Symptom management in collaboration with the patient's community physician and/or the hospice medical director on call.
-Listening and responding appropriately to patients/families and providing support in crisis situations
--Providing patient/family education.
-Assisting with coordinating care
The work schedule includes evenings/nights and a rotation requirement of every other weekend.

Minimum Requirements:
Valid NYS RN license
Associate's degree in nursing
Valid driver license and reliable vehicle
2 years + experience as a RN in an acute care environment
Preferred Requirements:
1+ year of experience as an RN in home care
Bachelor's degree in nursing
Strong understanding of Hospice principals
Ability to learn/use an electronic medical record
Maintain the highest level of customer service
Strong ability to assess priorities
Highest level of interpersonal skills to establish and maintain relationships with patients, families, physicians and coworkers
Ability to work independently
