Job Description:

Partners in Care is seeking a casual pysical therapist to join our team. The physical therapist plans, coordinates, implements and evaluates home health and hospice therapy services. The physical therapist is knowledgeable in providing therapy in the home setting. The therapist builds from the resources of the organization and community to provide services to meet the needs of individuals and families within their homes. The Physical Therapist is responsible for cost effective and effective patient care with quality outcomes.

Partners In Care is the oldest and finest hospice and home health organization in Central Oregon, emphasizing respect, compassion, and support within a family centered, full-service program. Partners In Care provides post-acute care to residents throughout a 10,000 square mile region of Central Oregon who are medically fragile, experiencing chronic and serious illness, recovering from surgery, or need end-of-life care at home or in our hospice in-patient facility called hospice house. As an independent non-profit organization, Partners In Care works in close partnership with the patient or clients family, the community, and other care organizations to ensure quality care.

Completes an initial comprehensive (if HH includes OASIS) and accurate assessment of patient and family to determine home health needs.

Develops an individualized care plan, which establishes measurable goals based on the therapy assessment including the patients functional abilities and limitations, rehab potential, and the patients diagnosis.

Collaborates with the patient/family and physician in the planning process.

Makes necessary revisions to the care plan as patient status and needs change and updates the physician as appropriate.

Identifies patient family needs for other home health services as necessary.

Initiates appropriate preventative and rehabilitation treatment and procedures.

Provides physical therapy instructions and HEP education to patient/family/caregivers as appropriate. Adapts teaching styles to meet patient
and family needs.

Actively promotes and participates in Fall Intervention Program for patients.

Ensures that arrangements for equipment and other necessary items and services are available and provided.

Identifies discharge-planning needs as part of the care plan development and implements prior to discharge of patient.

Delegates appropriate plan of care to licensed physical therapist assistant. Supervises licensed physical therapist assistant at least every 60 days and more frequently as stability of patient condition/progress warrants.

Develops, monitors, and updates the Home Health Aide Plan of care as necessary.

Instructs supervises and evaluates home health aide care provided every 14 days.

Communicates and updates the patients plan of care If needed when patient status changes.

Communicates with the physician regarding the patients needs, and reports changes in the patients condition; obtains/receives physicians orders as required.

Communicates and coordinates with all members of the healthcare team to coordinate the care plan including community resources.

Attends staff meetings, case conferences and IDT as required

Adheres to the PIC mission and values. Supports the organization.
