JOB SUMMARY:

The Licensed Social Worker will facilitate and coordinate the transition of patient care through the healthcare continuum. They will also direct and coordinate the integration of Social Services functions into the patient care, discharge, and home planning processes with other hospital departments, external service organizations, agencies, and health care facilities; in addition to providing direct care to selected patients as appropriate; and ensuring compliance with quality patient care. The LSW plans and supports programs to ensure compliance with regulatory requirements and accreditation standards in order to enhance the quality of patient care consistent with hospital policies for the adult and geriatric patients, 18 years and over. They will also provide clinical supervision and oversight for non-clinical social workers.

Education: Masters of Social Work required

Licenses/Certification: LCSW in the State of the District of Columbia

Experience: 3 years of clinical social work experience required including 1 year in healthcare setting
