Recovery Care Services seeks to strengthen and support individuals in recovery to live their best life. We achieve this by providing person-centered and recovery-oriented care coordination services that empower individuals through connections with a range of supports and treatments that optimize overall health and well-being.
The State Care Coordinator provides person-centered and recovery-oriented care coordination services that assist individuals who are transitioning from publicly funded and fee-for-service substance use treatment to the next level of care by connecting them with services that support their continuing recovery. This is accomplished through a compassionate and collaborative working alliance with individuals served for up to 6 months post-transition from a substance use treatment facility.
10. Utilize electronic records to capture all care coordination activities.
11. Build customer-oriented, collaborative relationships with treatment facilities and providers.
12. Enroll uninsured clients into available health insurance programs; assist them in choosing an MCO and identifying a primary medical
provider.
13. Perform other duties and assignments as assigned.
While performing the duties of this job, the employee is regularly required to talk, hear and respond professionally and courteously to internal and external customers and employees. The employee frequently is required to stand; use hands to finger, handle, or feel; reach with hands and arms; climb or balance; and stoop, kneel, crouch, or crawl. The employee is occasionally required to walk and sit. The employee must regularly lift and/or move up to 25 pounds and
occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. The employee must be able to safely operate their own vehicle to provide on-site community services.
*Float State Care Coordinator(s) must be able to provide on-site community services in multi-jurisdictions.
