The Community Hospice has a rewarding opportunity for a full-time RN to work as a part of our Admissions Team in Hospice Home Care working 4 10 hour days per week. Here at St. Peter's Health Partner's, we care for more people in more places.

What you will do:
Evaluate patients for eligibility and interest in hospice services, and describe the support and care available along with the benefits of hospice care.
Coordinate with the patient's community physician and our hospice medical directors to confirm eligibility for hospice services.
Completes the initial assessment of patient/family needs and begins implementation including ordering medications, medical equipment, supplies and support from our interdisciplinary team which includes social workers, chaplains, volunteers, complementary therapists, home health aides, and more!
Ensures a smooth hand-off to the Hospice Case Manager.
What you will need:
NYS RN Licensure
ASN required, BSN preferred
Homecare, preferably in Hospice, is strongly preferred. Hospice Homecare or 2 years acute care nursing experience
Reliable vehicle and driver license
Excellent communication skills. Must be able to communicate effectively with patients, physicians, and peers.
Must be able to document key information concisely and accurately
Strong computer skills
Strong organizational and time management skills
