RN Admissions-Full-Time-Albany Hospice

The Community Hospice has a rewarding opportunity for a full-time RN to work as a part of our Admissions Team in Hospice Home Care working 4 10 hour days per week. Here at St. Peter's Health Partner's, we care for more people in more places.

What you will do:

  • Evaluate patients for eligibility and interest in hospice services, and describe the support and care available along with the benefits of hospice care.


  • Coordinate with the patient's community physician and our hospice medical directors to confirm eligibility for hospice services.

  • Completes the initial assessment of patient/family needs and begins implementation including ordering medications, medical equipment, supplies and support from our interdisciplinary team which includes social workers, chaplains, volunteers, complementary therapists, home health aides, and more!

  • Ensures a smooth hand-off to the Hospice Case Manager.

What you will need:

  • NYS RN Licensure

  • ASN required, BSN preferred

  • Homecare, preferably in Hospice, is strongly preferred. Hospice Homecare or 2 years acute care nursing experience

  • Reliable vehicle and driver license

  • Excellent communication skills. Must be able to communicate effectively with patients, physicians, and peers.

  • Must be able to document key information concisely and accurately

  • Strong computer skills

  • Strong organizational and time management skills
