Community Health Worker/Care Coordinator- RCCC Program at Health Care Access Maryland

Job Description:

JOB PURPOSE

The Community Health Worker-Care Coordinator is responsible for providing outreach, social support, care transition, and community resource information to soon to be released clients and their families. The CHW-CC will provide services over a 60-day period, maintaining a caseload of up to 35 individuals at a time. The CHW-CC promotes access to medical and social services assisting clients in utilizing available resources; including scheduling appointments, accompanying clients to appointments as needed, educating clients about available community resources and referring clients to local social service agencies. Additional supportive services include assistance with completing forms and applications for benefits and community resources, coaching clients on self-management, conducting community visits, ensuring the client has active medical insurance coverage, and assisting the client with any COVID-19 concerns such a safe place to quarantine.  The Community Health Worker-Care Coordinator serves as a bridge for returning citizen client, the clinical care team and the community to assure the necessary services are available to address health-related social needs with a goal of reducing recidivism and emergency room use through coordinating these services.

ESSENTIAL DUTIES

The primary responsibilities of this position are:


  • Conduct social determinant of health screenings and health risk assessments prior to the clients release from custody.

  • Work as a team member within the State facilities with HCAM Benefits Specialists and Medical Discharge Planners and the medical/mental health teams

  • Document assessment, contacts, and services in electronic medical record (eCW)

  • Attend meetings held at Prison facilities, client homes/transitional houses/recovery houses, HCAM offices or other facilities as agreed upon by the Community Health Worker and client.

  • Effectively use motivational interviewing techniques to engage and coordinate care/services to clients.

  • Provide client centered services in a trauma informed approach

  • Connect with clients on a weekly basis for up to two months post release, to provide ongoing assistance accessing services and coordinating care.

  • Consistently achieve productivity goal and performance measures established for Community Health Worker-Care Coordinator

Additional duties and responsibilities:


  • Empower clients to engage, stay engaged, re-engage with community services.

  • Enroll uninsured clients into available health insurance programs; assist them in choosing an MCO and identifying a primary medical provider.

  • Refer to relevant programs that assist residents of Baltimore City, Baltimore County, or Anne Arundel County or any other Maryland jurisdictions as needed

  • Perform other duties and responsibilities as assigned

  • Attend all mandatory trainings and scheduled meetings.

  • Obtain Maryland State CHW Certification

  • Obtain DPSCS training to access Maryland State Institutions

  • CPR certification

  • Follow HIPAA, confidentiality, and mandated reporting.

  • Report to appropriate authority representative; in cases of abuse, neglect, or imminent danger.

  • Must be able to work flexible non-traditional hours with shifts on Monday-Saturday as schedule may shift depending on population served

QUALIFICATIONS:

Education, Experience, Certifications and Licensures:



  •           REQUIRED EDUCATION:

    • Minimum high school education or GED required- Bachelor’s degree preferred in Human/Social Services

    •  (6) months experience working in a Community Health Worker (CHW) role or in another position with the provision of social services, supportive services, personal care, or related duties                              



  • REQUIRED LICENSE:                


    • Valid Maryland driver's license and insurance required

    • Maryland Insurance Certified Application Counselor (must complete within 2 mo. of hire)

    • Maryland Community Health Worker Certification within 12 months of hire)

Knowledge, Skills & Abilities:


  • Strong computer literacy skills, specifically with MS Word, Excel, Outlook and SharePoint (365).


  • Detail oriented to complete requirements of files and contract compliance.

  • Sensitivity to cultural and socioeconomic characteristics of population served.

  • Excellent oral and written interpersonal and communication skills required

  • Proven ability to work efficiently and meet deadlines

  • Ability to be flexible and creative

PHYSICAL REQUIREMENTS

While performing the duties of this job, the employee is regularly required to talk, hear and respond to customers and employees. The employee frequently is required to stand; use hands to finger, handle, or feel; reach with hands and arms; climb or balance; and stoop, kneel, crouch, or crawl. The employee is occasionally required to walk and sit. The employee must regularly lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

WORK ENVIRONMENT

While performing the duties of this job, the employee is exposed to both indoor and outdoor conditions. The employee will be subject to noise.
