We are currently seeking a Supervisor I- Care Worker to serve members in Wake county.
The Supervisor I-Care Workers provides support, supervision, and development to Care Workers who collaborate in a team structure for the benefit of overall care management activities to include monitoring of service provision and member satisfaction with those services. They provide direct observation of Care Worker deliverables and monitoring of successful delivery through review of available reports.
This position will allow the successful candidates(s) to work a schedule which will include both onsite (in the county served) as well as remote work 4 days of the week as approved by their supervisor.
Responsibilities & Duties
Supervise and Develop Staff
Work with Human Resources and Unit Director to maintain and retain a highly qualified and well-trained workforce
Ensure staff are well trained in and comply with all organization and department policies, procedures, business processes. and workflows
Ensure the department has the needed tools and resources to achieve organizational goals and to support employees and ensure compliance with licensure, regulatory, and accreditation requirements
Actively establish and promote a positive, diverse, and inclusive working environment that builds trust
Ensure all staff are treated with respect and dignity
Ensure standards are transparent and applied consistently, impartially, and ethically over time and across all staff members
Work to resolve conflicts and disputes, ensuring that all participants are given a voice
Set goals for performance and deadlines in line with organization goals and vision
Effectively communicate feedback and provide ongoing coaching and mentoring to staff and support a learning environment to advance team skills and professional development
Cultivate and encourage efforts to expand cross-team collaboration and partnership
Collaborate with Alliance Care Team and Provider Networks
Collaborate and coordinate care management support practice standards across all populations through routine consultation with Care Workers. In addition, cross functional collaboration is expected among supervisors of all Care Management programs
Monitor and manage successful internal and external stakeholder relationships to achieve care management deliverables for members
Exercise conflict resolution skills to appropriately resolve issues with internal stakeholders, ensuring that all parties are given a voice
Collaborate with Providers and External Stakeholders
Develop strong working relationships with providers and external stakeholders by scheduling ongoing opportunities to share feedback and collaborate
Participate in local, regional, and state meetings as required
Exercise conflict resolution skills to appropriately resolve issues with providers and external stakeholders
Participate in team meetings with members, stakeholders, and others as needed
Make recommendations for Continuous Quality Improvement
Make recommendations to improve department procedures and increase operational efficiency
Monitor trends and identify opportunities for enhancements in service utilization and implementation throughout Alliance
Maintain Documentation
Maintain medical record compliance/quality
Ensure timely documentation of Care Coordination activities as required by department policy and procedures
Monitor completion of documentation in all systems to ensure data is entered timely and accurately
Comply with Policies, Procedure, Processes, and Workflows
Adhere to all Alliance Organizational Policies and Procedures and Care Management Desk Procedures
Minimum Requirements
Master’s degree and three (3) years of experience with the population served,
Or
Bachelor’s degree in a Human Services or related field and five (5) years of experience with the population served
Or
Graduation from an accredited nursing school and current registered nursing license and five (5) years of experience with the population served.
Preferred Requirement-Supervisory Experience
Experience with I/DD and/or TBI is a plus
Special Requirements
Requires current NC Drivers License
Certification in NACCM, NADD-Specialist, and/or CBIS required within 12 months of hire
Knowledge, Skills, and Abilities required
Knowledge of Medicaid basic, enhanced MHSUD, and waiver benefits plans
Knowledge of and skilled in the use of Motivational Interviewing techniques
Strong interpersonal and written/verbal communication skills essential, including conflict management and resolution.
High level of diplomacy and discretion is required to effectively negotiate and resolve issues with minimal assistance.
Strong problem solving, negotiation, arbitration and conflict resolution skills are essential to balance the needs of both internal and external customers.
Ability to understand macro and micro level planning, maintaining both the big picture and seeing that the details are covered.
Detail oriented
Ability to organize multiple tasks and priorities.
Ability to effectively manage projects from start to finish
Ability to adapt and shift focus according to mandated changes and changing priorities within the department.
Proficiency in Microsoft Office products (such as Word, Excel, Outlook, etc.) is required
Demonstrated ability to be flexibility and adaptable to change.
Ability to Assess and conducting safe home visits
Knowledge of regulations and statutes specific to 1915(b) and (c) waiver services including licensure type required for facility-based services, and staffing and supervision requirements
