TheLEAD Case Manageris responsible for developing and implementing outreach and engagement strategies for individuals at risk for being arrested for low level offenses. Coordinate with diversion trained law enforcement and provide services for the engagement and sustained engagement of individuals who have need case management to avoid incarceration. Coordinate screenings, assessment, and referrals to the various partner organizations of the CoC.Provide case management, linkages, coordination, advocacy and supports needed by client. The position will demonstrate knowledge of health care, social services, employment, housing, recreational opportunities, and other services and systems available in the community.
Pay: $40,000 to $50,000 annually
