AHealthy Transitions Facilitatoris needed in the Department of Behavioral Health for theHealthy Transitionsprogram.This position provides a wide variety of support services designed to improve successful transitions fortransition-aged youthin the areas of employment, education, living situation, and mental health services. The Healthy Transitions Facilitatorwill work with clients in their home and throughout the community to provide life skills training, case management services or referral services to ensure clients needs are met. The Facilitator will maintain an outcome focus and will involve young people, parents and community partners in the delivery of service.
