Essential Duties Wellness Nurse - LPN::

Assist with coordination of resident care
Coordinate resident outside services
Mentor and provide support to CNAs
Thoroughly document and follow-up
Provide support to residents and their families
Wellness visits within our community
Assist with medication management
Communicate and coordinate with the DON/ADON
