The Nurse Coordinator provides education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum. The Nurse Coordinator will be the point of contact for a specific group of patients, including hematologic malignancies, solid tumors, and/or breast cancer patients.
Essential Duties and Responsibilities
Coordination of Care
Perform comprehensive nursing assessment of patients within scope, in clinical areas, and via telephone in collaboration with MD/NP/PA providers
Communicate with MD/NP/PA regarding patient needs in a clinically appropriate and timely manner.
Participates in coordination of the plan of care with the multidisciplinary team
Collaborate with Infusion Center nurses and staff in the care and treatment of patients.
Serve as a liaison between the physician and the patient/family for home and hospice care.
Education of patients and caregivers
Assess educational needs patients and support system by taking into consideration barriers to care, and current clinical needs
Provides and reinforces education to patients and support system about diagnosis, treatment options, side effect management, post-treatment care, and survivorship
Orients and educates patients and caregivers to the cancer healthcare system, multidisciplinary team members and roles and available resources
Offer patient, family, and caregiver education related to symptom management, diagnosis, and treatment; develops and implements patient education materials related to same.
Educates and reinforces the significance of adherence with patients and caregivers regarding treatment schedules, protocols, and follow up

Education
Bachelor of Science in Nursing preferred
Requires DC Nursing License
OCN/BMTCN certification required within 12 months of hire date.
Experience
Minimum of two years experience as an oncology nurse, familiarity with vascular access devices
Typing and computer skills necessary
Good interpersonal communication skills
Able to work effectively with medical professionals at all levels including Hematology/Oncology Clinic Staff, administrative personnel in the Division of Hematology/Oncology; Unit personnel, and Social Service Workers
