POSITION SUMMARY:


This role facilitates individual and/or family therapy sessions which may include, parent coaching, and crisis intervention for each child/family/adult assigned to his/her caseload. This position creates the treatment plan, crisis/safety plan and collaborates with and/or directs an interdisciplinary team.


GENERAL DUTIES/RESPONSIBILITIES:


• Provides clinical leadership regarding the ordering of treatment priorities and frequency
• Assesses and prepares a comprehensive integrated service treatment plan based on the strengths and needs of the consumer including but not limited to:
• Diagnosis and current mental status of the consumer.
• Psychosocial evaluation of the consumer family, including psychological. social,
vocational and educational factors important to the consumer family and the dynamics
within the consumer family unit
• Creates treatment plan to specific goals for the consumer and other members of the consumer family. Goals shall include but not be limited to
• Short-term, realistic, specific objectives related to each goal and described in terms of specific measurable outcomes and timelines.
• The person responsible for carrying out each part of the plan.
• The activities or modalities to be employed.
• Objectives which are evaluated and redefined periodically according to the progress made in attaining the objectives.
• Delivers advanced individual, family and group therapy and counseling, sensitivity training, play therapy, recreational therapy, cognitive techniques, parenting skills, assertiveness training, reality therapy, rational/emotive therapy, modeling, behavior modification and coping skills.
• Facilitates or co-facilitates crisis intervention
• May provide on-call services as scheduled, including individual and family support, face to face contact, and assistance with assessment for inpatient hospitalization or bed searches (payor requirement in some counties) as needed.
• Monitor progress, regression or consistency with goals outlined in Integrated Service Plan
• Completes accurate and timely documentation per policy
• Performs case management duties, as needed.

Master's degree in Psychology, Counseling or Social Work plus 1-3 years of experience.
