The Patient Care Manager is a professional, registered nurse responsible for assisting the Clinical Director with hospice clinical services. 


Job duties of this rewarding position include:



  • In the absence of the Clinical Director the Assistant Clinical Director will step in the role of the Clinical Director and will be vested with authority to act on behalf of the Clinical Director.

  • Maintains current knowledge related to hospice safety and infection control standards.

  • Collaborates with nursing supervisors to provide a process to promote and ensure employee orientation, competency and in-service education programs.

  • Ensures proper maintenance of clinical records in compliance with local, state and federal laws.

  • Assists with the maintenance of adequate and appropriate inventory supplies and equipment for
    the provision of patient/client services.

  • Ensures the quality and safe delivery of hospice services provided through the Organization.

  • Ensures compliance with all local, state and federal laws regarding licensure and certification of
    organization personnel and, maintains compliance to the ACHC Hospice standards.

  • Stays informed about changes in the field of nursing and hospice care; shares information with
    appropriate organization personnel.

  • Leads Interdisciplinary group and the clinical staff directly.

  • Assists in the identification of skills for annual competence and provides in-service training support.

  • Maintains aggregate records of employee orientation and annual competency requirements as part of the QAPI plan.

  • Monitor organization compliance with regulatory and accreditation standards.

  • Maintains current knowledge related to Medicare Conditions of Participation and the Accreditation Commission for Health Care’s (ACHC) standards to ensure that the organization is compliant with state, federal and accreditation guidelines.

  • Responsible for ensuring policies and procedures reflect all related local, state, federal and ACHC standards.

  • Acts as a resource to Clinical Director regarding standards, policies, regulations and provides in-service training to leadership and staff as needed in these areas.

  • Acts as a clinical resource to staff.

  • Maintains updated nursing knowledge and skills.

  • Assists with patient/client visits as needed.

  • Assists with administrative call on a rotational basis and clinical call as needed.

  • Promote self-growth and collegial relationships with others in the hospice industry.

  • Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal and professional networks and participates in professional organizations or societies.

  • Demonstrates C.A.R.E. values to our patients/clients, family members, customers and staff.

  • Comply with the agency’s privacy practices and procedures related to patient/client and employee records and all state/federal health privacy laws as outlined by HIPAA.

  • Demonstrates teamwork and prompt and regular attendance to work to ensure that quality care and services are provided to the patients/clients we serve.

  • Complies with and adheres to the appropriate use of Personal Protective Equipment (PPE) required by the Bloodborne Pathogens Standards. Protective Personal Equipment (PPE), including personal protective equipment for eyes, face, extremities, protective clothing, and protective shield and barriers, will be provided, used, and maintained.



Job Requirements:




  • Registered Nurse (RN) with current licensure
    Bachelor's or Associates degree in Nursing

  • Excellent understanding of performance improvement, quality assurance, and utilization management.

  • Three (3) to five (5) year's clinical nursing experience (An emphasis in hospice nursing is preferred).
