Job Description:


Summary:

The Program Manager for Mary Free Bed at Sparrow's Day Rehabilitation Program is responsible for the overall operation of this outpatient, interdisciplinary rehabilitation program. The Program Manager collaborates with the Director of Rehabilitation, Medical Director, and other leaders and staff from multiple disciplines to design, implement, evaluate, and adapt systems of care to achieve targeted performance outcomes.

Working with other team members and other members of the Sparrow and Mary Free Bed leadership teams, the Program Manager is responsible to organize, direct, and ensure effective operations of all services provided in the day rehabilitation program.

The Program Manager oversees the interviewing and selection of staff, the corrective action process, and completes formal evaluations of employees. The Program Manager uses independent judgement in problem solving, takes necessary steps to resolve issues, organizes work flow, assigns employees and participates in budget planning and compliance.

Essential Job Responsibilities:

  1. Quality Care - The Program Manager's primary focus is creating a team and processes that contributes to the delivery of quality outpatient rehabilitation care. The Program Manager collaborates with other members of the leadership team to:


    1. Promote clinical excellence through applied clinical knowledge, leadership, and evidence-based research utilization.

    2. Orchestrate change to improve clinical and organizational outcomes.

    3. Integrate continual performance improvement into the structure and processes of the daily practice of team members to achieve desired clinical and fiscal outcomes.

    4. Align all clinical activities with organizational and department performance improvement goals, encouraging collaboration and problem solving across the various disciplines.

    5. Collaborate with other disciplines to design, implement, evaluate, and adapt systems of care within the day rehabilitation program to achieve targeted clinical quality and fiscal outcomes.

    6. Apply a systematic process for program planning and resource allocation for populations of patients.

    7. Ensure that the physical environment, patient confidentiality, and patient rights are maintained promoting safe quality patient care outcomes.

    8. Ensure that patient care documentation is complete and accurate in accordance with hospital policies and procedures.

    9. Promptly report actual or potential adverse events or safety issues.

    10. Prepare for and be ready for regulatory reviews: State; Fire Marshal, Joint Commission, CARF, etc.

    11. Work with all staff to assure that services are delivered consistently according to established policies, procedures, and protocols. This is accomplished by creating equitable staffing, mentoring new staff, ensuring staff presence at and preparedness for team conferences, communication to and follow up with physicians, safety monitors, and timely follow up on occurrence reports as well as customer service recovery as needed.

    12. Develop and review therapy policies and procedures; standards of practice, treatment, and services.





  1. Key Performance Indicators - The Program Manager is responsible to understand performance measures for the day rehabilitation program, and to collaborate with other rehabilitation and hospital leaders and staff to identify opportunities for improvement and develop action plans.



  1. Quality Improvement - The Program Manager takes an active leadership role in Mary Free Bed at Sparrow's Day Rehabilitation quality improvement program.



  1. Patient Safety - The Program Manager is responsible to create an environment of care that promotes the safety of each patient and team member. Using feedback from staff and other members of the leadership team, the Program Manager will assure that appropriate steps are taken to protect the safety of each patient while they are receiving therapy.



  1. Interdepartmental and Interdisciplinary Collaboration - The Program Manager is responsible to maintain effective interpersonal communication and working relationships with all leaders, staff, physicians, and other key stakeholders. The Program Manager will lead interdisciplinary program meetings with the goal of evaluating and improving the care provided at Mary Free Bed at Sparrow Day Rehabilitation.



  1. Accreditation and Regulatory Responsibilities -The Program Manager works with the Director of Rehabilitation Services and other leaders to implement policies, procedures, and processes that align with regulatory, accreditation, and state standards, when applicable.

  1. Customer Service - The Program Manager is required to demonstrate excellent customer service standards and behaviors, while interacting with patients, family members, physicians, and colleagues. The Mary Free Bed Standards of Behavior outlines expectations for performance in this area.

The Program Manager must deal discreetly and sensitively with confidential information, and as needed, will perform customer service recovery with patients and families members. The Program Manager will also take active steps to assist staff members to recognize unique cultural and religious needs of patients and families and integrate these diverse needs into a patient-specific plan of care that aims to exceed the expectations of each patient and family. Finally, the Program Manager will hold team members accountable for performance in all aspects of customer service.



  1. Physician and colleague relationships - Interdisciplinary team integration and collaboration across disciplines to provide patient-centered care is a priority at Mary Free Bed at Sparrow. Maintaining effective working relationships with physicians is an important responsibility of the Program Manager. The Program Manager is responsible to develop and maintain professional working relationships with colleagues from other disciplines that are involved in the care of each patient and family. When challenging situations are identified by staff members, the Program Manager will assist with problem solving and service recovery, as appropriate.

The Program Manager oversees various personnel actions including but not limited to, hiring, performance appraisals, disciplinary actions, employee recognition, terminations and other related activities.

Business Development - The Program Manager is responsible for all aspects of business development for the program, working in collaboration with the Director of Rehabilitation Services and colleagues from the marketing and business development departments from both Mary Free Bed and Sparrow.

Referral Relationships - The Program Manager is responsible to facilitation strong relationships with key referrals sources, from within Sparrow Hospital and with other referring physicians and case managers in the region. This includes assuring that the referral response process is timely, efficient, and consistent, thereby meeting the needs of patients, families and referral sources.



  1. Marketing - In collaboration with the marketing teams from Sparrow and Mary Free Bed, the Program Manager will provide input into the marketing strategies for the MFB at Sparrow Day Rehabilitation Program, and will oversee the implementation of tactical steps to promote the services provided.

Fiscal Responsibility - The Program Manager will implement departmental policies and procedures that align with Mary Free Bed and and/or Sparrow policies related to payroll practices, patient chargeable items and other opportunities to provide high-value care at a reasonable price to our customers.     2. Supports the organizations patient flow efforts through compliance with rehabilitation hospital policies.

Essential Job Qualifications (Knowledge, Education, and Training Requirements):



  1. Licensure: Licensed as a Registered Nurse, Physical Therapist, Occupational Therapist or Speech Language Pathologist in the state of Michigan.


  2. Education: As required to obtain licensure in Michigan.

  3. Current Basic Life Support (BLS) certification


  4. Experience:

    • Previous experience as rehabilitation leader is required.

    • Strong critical thinking, decision-making, delegation, and leadership skills are required.

    • Time management and organizational skills are very important, as well as effective written and verbal communication skills.

    • Must be able to work effectively under time requirements and deadlines, problem-solve when facing unexpected issues, handle interruptions, and prioritize effectively.





  1. Physical Demands/Work Environment: The work environment described here are representative of those an employee will encounter while working as a Program Manager at Mary Free Bed at Sparrow. The ability to perform these tasks are required in order to provide the necessary patient care required by MFB at Sparrow patients:


    • Required to stand, sit, talk, type, read, and hear

    • Required to use hands and arms to handle, feel, carry and work

    • Required to frequently stand, walk, bend, kneel, and rise from a kneeling position while performing patient care.

    • Required to be independently mobile, with frequent bending, and reaching overhead, for activities such as positioning and transferring of patients and equipment.

    • Must be able to exert 20 to 35 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects(*).

    • Due to possible exposure to blood-borne pathogens and hazardous materials, must be able to understand and follow department policies related to employee safety.

    • Must be competent with computer systems for the purpose of documenting patient care.
