GENERALSUMMARY/ OVERVIEW STATEMENT:
Treats patients at North EndWaterfront Health within Behavioral Health. Promotes a collaborative relationship andeffort between the medical staff, NEW Health staff toward continuity of patientcare and efficient use of resources. Serves as a role model for professionalnursing practice.
PRINCIPALDUTIES AND RESPONSIBILITIES:
Providesdirect care to patients at North End Waterfront Health in collaboration withthe physician. Performs and records the patient history and physical exam anddiscusses findings with the Physician. Utilizes advanced nursing knowledge toidentify, prevent or solve complex and/or recurring patient care problems.Follows and manages patients (in collaboration with MD) enhancing continuity ofcare. Performs (oversees/assures completion of) specialized procedures particularto North End Waterfront Health population mutually agreed upon withcollaborating physician. Performsmedically delegated aspects of patient care as agreed upon by nursing andmedical administrators according to practice guidelines. Collaborates with unitstaff and physicians to facilitate a comprehensive plan of care and coordinateresources. Writes orders which are in accord with current guidelines. Maintains and practices in accord withpractitioner guidelines per state and hospital regulations.
Provides consultation to both nurses andphysicians. Serves as a resource to staffin area(s) of expertise. Serves onappropriate intra and interdepartmental committees. Participates in the development of formal andinformal educational programs for the unit-based nursing staff. Develops educational programs within area ofexpertise. Collaborates with unit staff,clinical teachers, clinical specialists, staff specialist and other health careprofessionals to meet staff and patient educational needs.
Monitors patient outcomes in accordance with definedstandards of patient care. Collaborateswith nurse managers, clinical nurses and medical staff to plan, implement andevaluate pertinent change in practice and health care delivery. Identifies system-wide factors contributingto undesirable outcomes, such as complications, length of stay andover-utilization.
Facilitates patient movement through the system andefficient use of resources through interdepartmental collaboration. Supports research efforts to advanceknowledge and to promote research-based practice. Evaluates own practice and programeffectiveness. Identifies pertinentnursing and health delivery problems for investigation. Evaluates current research findings forclinical specialty and determines applicability to unit practice.
Responsible for own professional growth andmaintenance of required credentials. Maintains the standards and educational requirements specific to theposition and licensure. Identifies ownlearning needs and goals. Maintains andupdates clinical knowledge and skills based on current health carepractice/research.
