Job Description:

GENERALSUMMARY/ OVERVIEW STATEMENT:



Under the direction of theiCMP Medical Director and the Senior Administration Director with DGIM, the ProjectManager supports the development and execution of efforts to create andmaintain systems that coordinate care for high risk patient populations whoreceive primary care at MGPO/GH. S/heworks closely with senior leadership, the iCMP Clinical Director, Primary Care leadership,the Department of General Internal Medicine, and MGPO/GH Population HealthManagement to help formulate strategic plans to improve care delivered to thehighest risk patients in the Medicare, Medicaid and identified commercial payerpopulations. Collaborates with numerous MGH departments and Mass GeneralBrigham Population Health Management, in order to help iCMP meet populationhealth management program requirements, contractual targets, and MGPO/GH/DGIMpractice standards.


PRINCIPALDUTIES AND RESPONSIBILITIES:




  • Supports thedevelopment, implementation and as needed ongoing administration of the iCMP atMGH that coordinates care for identified high risk Medicare, Medicaid andcommercially insured patient populations who receive primary care at theMGPO/GH.


  • With the iCMPManagement Team, responsible for programmatic oversight and programdevelopment/enhancements.


  • Helps to ensurethat programmatic projects and activities are complete and on time.


  • Helps to ensurethat iCMP projects align with DGIM and MGH programs, projects, and priorities.


  • Manage themonthly iCMP team meeting.


  • Responsible forprogram offices including central space at 125 Nashua Street and the staffspace in primary care practices.


  • Co-Oversees theiCMP budget. .


  • With the iCMP Medical Director and Clinical Director, represent MGH at MassGeneral Brigham iCMP Operations Management, Project Managers and other ad hocmeetings.




  • Assures iCMP SharePointsystems and website are maintained.


  • In collaborationwith iCMP management team, supports the hiring and training of clinical staff(e.g., information systems, hr forms, space)





  • Assist in programpresentations including development of PowerPoint slide decks.


  • Attends andparticipates in meetings and committees within the Hospital and community tokeep abreast of current issues that may affect the MGPO/GH/DGIM.


  • Supports anddemonstrates the values of the MGPO/GH/DGIM by conducting activities in anethical manner with integrity, honesty, and confidentiality. Demonstrates a positive open minded, can doattitude. Represents a team perspectiveand willingness and enthusiasm to collaborate with others. Follows through on commitments and achievesdesired results. Exhibits soundjudgment, obtains the facts, examines options gains support and achievespositive outcomes


  • Performs otherduties as assigned.







Qualifications

QUALIFICATIONS:





  • Master's degreein public health, healthcare administration, planning and/or policy or relatedfield preferred.


  • Minimum of 3-5 years'experience in healthcare field, preferably in an Academic Medical Centerenvironment.


  • Previousmanagement, operations and/or program development skills required.







SKILLS/ABILITIES/ COMPETENCIES REQUIRED:





  • Ability to manageexpansive program(s) within a complex matrix organization, i.e. to persuade andengage physicians and staff at all levels of the organization.


  • Excellent oraland written communication skills and oral presentation skills.


  • Demonstratedknowledge of quality measurement.


  • Track record inmanaging relationships and building networks.


  • Experience with MicrosoftPowerPoint, Excel and Word, Share Point.


  • Analytic abilityand skills.







WORKINGCONDITIONS:





  • Ability to workwell with others in a demanding and changing environment.


  • Office-basedenvironment. Frequent travel to variousbuildings on the MGH campus and occasional travel to other Mass General Brighamfacilities.







SUPERVISORYRESPONSIBILITY:



  • Ability to enlistand motivate others without direct supervisory authority.


  • Provide oversightfor related contracts for outside services.








  • FISCALRESPONSIBILITY:





    • Monitor iCMPprogram budget
