The Case Manager is responsible for facilitating and driving the progression of care for the patient's hospitalization from preadmission through discharge. The Case Manager coordinates with physicians, nurses, social workers, and other health team members to expedite medically appropriate, cost-effective care. The Case Manager applies clinical expertise and medical appropriateness criteria to resource utilization and discharge planning. Will advise the healthcare team and provide leadership as needed.

High degree of skill in written/oral communications and personal interactions. Demonstrates agility in a continuously changing fast paced work environment. Demonstrates computer competency. Ability to complete daily job responsibilities within normal hours by moving rapidly through the hospital and physician offices. Must be able to perform the essential job functions, either unaided or with the assistance of a reasonable accommodation to be determined by the organization on a case-by-case basis. Culturally competent. Valid drivers's licensed and reliable automobile.
Required:2+ years of appropriate clinical experience in utilization review or discharge planning experience preferred.
Preferred: 4+ years hospital level care and discharge planning.
Completion of an accredited Nursing program.
Current Nursing in the State of MA is required.
