Job Description:

Position Summary:
The Registered Nurse (RN) reports to the Hospice House Inpatient Supervisor. In collaboration with the Interdisciplinary Team, the RN is responsible for providing nursing care, support and education to terminally ill patients and their family members in accordance with agency policies. The RN assists in ensuring continuity of care, cost effectiveness and quality of clinical operations while facilitating communication between leadership and facility staff.
Functions:

Applies nursing principles and techniques in the treatment and care of terminally ill patients with a focus on Hospice Care and symptom management.
Assesses continuing educational needs and provides ongoing education and emotional support to patients, families and caregivers.
Is responsible for the implementation and continued development of the patient plan of care (PPOC) in collaboration with Interdisciplinary Team (IDT).
Is responsible for completion of admission, case management and discharge of patients in collaboration with the Interdisciplinary Team (IDT).
Documents care accurately within the patient's Medical Record in accordance with agency policies.
Communicates with members of the Interdisciplinary Team (IDT) as needed regarding patient status and to coordinate and ensure consistent high quality care.
Follows the guidelines for universal-precautions when providing patient care.
Supervises and collaborates with Licensed Practical Nurses (LPNs) and Certified Nurse Assistants (CNAs).
Serves as the charge/resource nurse when assigned, coordinating staff and patient care and assisting in issue resolution.
Attends and participates in daily report, team meetings, briefings, in-services, department and staff meetings and other related activities.
Understands and adheres to the agency's mission, policies and procedures.
Works effectively within an Interdisciplinary Team (IDT) environment
Demonstrates a positive attitude and partners effectively with co-workers, patients, families, health care professionals, donors, and other community partners, consistently representing the facility to all internal and external customers with integrity and professionalism.
Prioritizes, manages, and organizes work to complete essential job duties, meet performance objectives, and effectively practice time management.
Carriers out duties and responsibilities of position in a manner that exemplifies excellent customer service.
Maintains strict confidentiality of patient, family and caregiver information.
Provides for a safe environment for patients, guests, visitors and employees.
Adheres to all policies, procedures and regulations regarding patient care, conduct, safety, fire, security and risk management
Maintains positive work relationships and contributes to the team environment by:

Demonstrating respectful and appropriate language and nonverbal behavior,
Actively seeking to resolve customer and co-worker conflict issues, problems and concerns in a timely manner
Demonstrating respect, courtesy and empathy to all customers, patients, families, co-workers, physicians and volunteers.

Assists in ensuring adequate staffing levels for all shifts within the Hospice Houses by:

Floating shifts between Kansas City Hospice House and North Care Hospice House on a rotational basis with other Hospice House employees.
Work a temporary, alternative schedule or shift as requested by supervisor.

Other duties as assigned.
