This position is from 4:30pm - 8:30am Monday - Thursday, Friday 4:30pm - Monday 8:30am. The schedule is 7 days on and 7 days off. Key Job Responsibilities: Responsible for triaging and resolving issues for Hospice. Maintaining the Home Care/Home Base database for accuracy. Responding to the needs of the patients, families and physicians calling into Triage with any issues. Schedule field staff in an accurate and timely manner.
Responsibilities:
Triage patients requiring urgent and emergent medical advice evenings, weekends and holidays via telephone.
Process incoming hospice referrals
Assign Admission, evaluation, subsequent, and discharge visits within EMR.
Reassign Admission, evaluation, subsequent, and discharge visits within EMR.
Run scheduling reports.
Assist field staff with ordering durable medical equipment (DME) and patient supplies as needed.
Assist with start of care admission process, displaying proficiency in the identification of hospice eligibility requirements set forth by Medicare guidelines.
Educate both the patient and family on medication management, treatment protocols and end of life processes along with identifying comprehensive and safe understanding of caregiver ability to provide safe care.
Engage in effective interdisciplinary communication with various professions of the hospice care team to promote the most effective care for the patient and their family.
Proficiently document all health assessments, physician orders and communication with the patient, their family and various medical professionals in a timely manner.
Maintains patient confidentiality, including all HIPAA regulations.
Compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety and emergency management.
Manages Territory with minimal supervision based on forecasted needs, priorities and business strategy.
Professional Experience and Qualifications
1+ year of previous Hospice nursing experience.
Minimum of one year of experience in medical/surgical and one year of experience in critical or emergency care with specialized triage training.
Strong written and verbal communication skills.
Excellent customer service skills.
Commitment to The Care Team's philosophy of care, values, vision and mission statements as an exemplary role model for all staff.
Must have at least one-year experience in the Hospice industry.
Knowledge of Home Health is preferred.

Regulatory Requirements
Graduate of an accredited school of nursing, with a bachelors degree preferred.
Current state license as a registered nurse or Licensed Practical Nurse.
Must pass a criminal background check & MVR check.
Completed health statement acknowledging ability to perform the duties of the position.
